Job Title: Care Management Administrator Location: Monroe County
Reports to: Program Director Dept.: BHS
Employment Type: Full Time Hours:Mon-Fri 8:30-5:00
Description:
To assist in Health Home Care Management program functions and programmatic needs. The administrator is the hub of all referrals, assignments, caseloads and monitors maintenance of electronic records. This is an essential role in ensuring the program meets all compliance and quality assurance policies and guidelines set forth by HHUNY and DOH
Duties and responsibilities:
- Maintain reports and monitor movement of clients from intake to discharge i.e., client search, enrollment, diligent search efforts (DSE), excluded setting, transfer, and discharge.
- Track specialties of care such as HARP and Health Home Plus (HH ) using MAPP, and make necessary updates to reflect changes in client charts.
- Assist in facilitating referrals to ensure communication and coordination between department and community-based organizations.
- Maintain client chart compliance records and prepare reports as requested to assist in management of workflow.
- Working closely with program director, the administrator supports the implementation of quality improvement measures for the care management program and staff.
- Participate in HHUNY trainings and webinars as assigned by the program director.
- Working closely with the program director, the administrator supports program growth and development.
- Using internal program trackers, reports, and data, the administrator works with staff to ensure client charts are completed correctly and meet Health Homes Care Management program and Department of Health requirements.
- Participate in annual internal/external audit processes
- Perform administrative support tasks to ensure program sustainability.
- Work with administrative assistant to maintain client records and database.
- Duties may change or be added by program director.
Minimal-Qualifications:
- Associate degree in human services field with 4 years of experience; or bachelor’s degree in health and human services preferred and minimum of 2 years of qualifying experience.
- Work history and education to include case management or casework with individuals who have chronic Illness, mental Illness, chemical dependency, developmental disability, homelessness, crisis intervention, etc.
- Computer proficiency is required. Must have excellent organizational and time management skills.
- Must have NYS Driver's License with no major violations within the last 5 years
