B&B Care Services, Inc. is looking for a Case Manager for our Elderly and Disabled Waiver Program (EDWP) to join our team. This position is a remote position that requires routine travel.

Responsibilities:

• Develop comprehensive 30/90 day care plans and update annually or as needed.
• Perform monthly contact assessment in consultation with the client/caregiver.
• Review financial, medical, and social information of applicant as presented by referral source.
• Conduct personal contacts with each client monthly, by phone or quarterly site visits.
• Verify Medicaid eligibility and/or screens for MAO/PMAO eligibility, using standardized guidelines.
• Explain the scope and purpose of the Traditional/Enhanced Elderly and Disabled Waiver Program (EDWP).
• Identify client needs and desired services as stated by the referral source or applicant.
• Research and maintains up-to-date knowledge of community services.
• Participate in case conferences with nursing team.
• Broker or assist in brokering Traditional/Enhanced EDWP services.
• Participate in the orientation of new personnel, in-service training programs, case conferences and staff development meetings.
• Ensure that patients and caregivers understand the patient's rights and responsibilities.
• Utilize time management techniques when scheduling and planning daily work activities.
• Perform other related duties as assigned.

Education & Experience:

• Bachelor's Degree in Social Work, Psychology or related field of study.
• Minimum of two years experience in a human service or health related field.
• Current certification in CPR and First Aid.
• Reliable transportation with current drivers' license and proof of liability insurance.
• Satisfactorily complete a criminal background check and drug screening.

Job Type: Full-time

Pay: From $25.00 per hour

Expected hours: 40 per week
