**SALARY- Up to $110K per year (Depending on Experience)**
A beautiful privately owned skilled nursing facility is seeking a strong and compassionate Director of Social Services.
Requirements of the Director of Social Services:
- Responsible for all admission and discharge aspects of facilities sub acute and long term population.
- Coordinate the admission of and assist in the initial adjustment for patients and their families/support unit.
- Provide on-going social work services to patients and significant others that conform to regulatory and professional standards.
- Coordinate the discharge planning with interdisciplinary team including therapy, nursing, homecare agencies and reimbursement coordinators.
- Provide continuous communication to families for the duration of the residents stay.
- Must have excellent organizational skills
- Must be a team player
- Must have experience in long term care
Qualifications:
·Must have BSW or MSW, LMSW strongly preferred
·Must have prior experience as a Director of Social Services
·Must have long term care experience
·Strong leadership skills in a healthcare setting
·MDS experience preferred
·SIGMA or PointClick experience preferred
·Strong written and verbal skills
