TITLE: GRYD Prevention Lead Care Manager

REPORTS TO: Director of Prevention & Intervention/GRYD Prevention Program Manager

POSITION HIGHLIGHTS: 
Program: 
GRYD Prevention: In 2015, ACE assumed responsibility for the City of LA’s West Valley (Devonshire-Topanga) Gang Reduction/Youth Development (GRYD) program. This program serves youth, ages 10-15, who are gang affected or otherwise at risk of gang involvement. Through a phased program of participant engagement, potentially leading to cycles of secondary services, the program works to strengthen participants' family ties and other pro-social influences.
Position:The Lead Case Manager is responsible to supervise the Care Managers. This position works in collaboration with the GRYD Prevention Manager in providing all care management services to clients of the GRYD Prevention program according to the GRYD Prevention Policies and Procedures. This position will provide care management services for Primary Service and Secondary Service participants. The position acts as a liaison within the community for those organizations servicing GRYD Prevention clients. The Lead Care Manager serves as the GRYD Program Manager when the GRYD Program Manager is not available.

  • Recognizes appropriate boundaries and limitations on interactions with program participants and their families and the need to maintain professional standards and practices. 
  • Is familiar with standard approaches to accurate participant record keeping, including timely charting, notation formats, content limitations, and confidentiality practices.
  • Possesses knowledge of programs, regulations, and procedures regarding human services, social services, justice, and workforce development.
  • Knowledge of community/gang dynamics within the GRYD Zone.
  • Able to operate within ACE’s designated GRYD zone, exposing oneself to gangs found within the zone.
  • Able to work in assigned locations that may expose oneself to hazardous conditions or in extreme physical or discomfort or distress.
  • Ability to assess conditions and responsiveness to interventions.
  • Possesses necessary de-escalation skills to provide crisis and community violence intervention in the community.
DUTIES AND RESPONSIBILITIES
  • Attends and participates in all mandatory meetings.
  • Fosters positive relationships with community members, partner organizations, and local gangs.
  • Participate in cultural and community building activities/events.
  • Works to incorporate into organizational culture best practices for diversity, equity, and inclusion, restorative justice, and trauma-informed care. 
  • Performs outreach and marketing duties to recruit gang-impacted participants and educate the community on ACE services.
  • Carries out intake and eligibility determinations and makes appropriate referrals when the applicant is not eligible for ACE services; conducts participant assessments leading to enrollment; completes and updates participant service plans. 
  • Serve as a point of contact in addressing community violence issues and provides the necessary resources, skills to assist participants involved.
  • Arranges and participates in participants’ family meetings, service meetings, team meetings, etc. and coordinates wrap-around services with other providers.
  • Partakes in mediation and advocacy as necessary, conducting home visits in high-crime areas, hostile environments, courts, and with their probation officers. 
  • Provides safe passage across gang lines to gang-impacted participants to ensure the safety of participants to receive necessary services.
  • Organizes, coordinates, prepares, and delivers skills-based workshops for gang-impacted participants and their families.
  • Enters data into several participant databases; assists in the writing of periodic program reports.
  • Organizes, coordinates, prepares, and delivers AYIDA skills-based workshops for participants
  • Enters data into participant databases; assists in the writing of periodic program reports.
  • Provides case management for an average standing caseload of twenty-five participants.
  • Responsible for administering the Y-SET assessment tool.
  • Responsible for completing reports and inputting them into the GRYD ETO Database.
  • Responsible for assisting participants and their families in making referrals to partner organizations.
  • Responsible for organizing celebration activity meetings with participants and support systems.
  • Coordinate CM’s for the proper delivery of services which will include the allocation of caseloads and service need determinations. 
  • Coordinates services to GRYD clients with care managers.
  • Assists GRYD Program Manager in the supervision of daily GRYD activities and reports to Manager any pertinent information.
  • Participates in all GRYD community-based events including Saturday Night Lights and Fall Friday Night activities.
  • Carries out other tasks typically performed by Care Managers
  • Other duties as assigned 
QUALIFICATIONS:
  • Bachelor’s Degree in relevant field and 2 years of gang intervention experience; completed Community Violence Intervention training, or
  • Equivalent combination of training and experience
  • Spanish language skills, verbal and written preferred
  • Possess a current California Driver’s License, clean DMV record & adequate auto insurance. 
  • Must submit to a Department of Justice Criminal background check 
  • Initial TB employment clearance required
The anticipated pay range for candidates who will work in California is $28.85 to $31.41 hourly The offered pay to a successful candidate will be dependent on several factors that may include but are not limited to years of experience within the job, years of experience within the required industry, education, etc.
