Job description:
Alliance for Community Empowerment (ACE) is a nonprofit organization serving the West San Fernando Valley.
Our mission is to empower marginalized communities by providing education/vocational programming, personal enrichment, and supportive services through collaboration and community action.
TITLE: Gang Reduction & Youth Development (GRYD) Care Manager
DEPARTMENT: GRYD
POSITION STATUS: Non-Exempt
REPORTS TO: GRYD Program Manager
POSITION HIGHLIGHTS:
The Care Manager is responsible for providing services to participants of the GRYD program. This position will provide case management services for Family Case Management (FCM) Model and Transitional Client Services clients. This position will work closely together with the GRYD Community Intervention Worker (CIW) to better assist GRYD clients. This individual will be primarily responsible for case management services to participants and will also be involved with all aspects of the program including intake, case management, and making appropriate referrals. The position will act as a liaison within the community for those organizations servicing GRYD clients. ALLIANCE FOR COMMUNITY EMPOWERMENT IS AN EQUAL OPPORTUNITY AFFIRMATIVE ACTION EMPLOYER.
KNOWLEDGE, SKILLS, AND ABILITIES
EFFECTIVE COMMUNICATION:
  • Conducts intake and eligibility of clients and makes appropriate referrals when the client is not eligible.
  • Collaboratives are ongoing with management to develop a strategic plan to outreach the GRYD clients.
  • Assists and works closely with Community Intervention Worker (CIW).
  • Responsible to set up clients’ Initial meetings, Family Meetings, Team Meetings, etc.
  • Attends and participates in all mandatory GRYD meetings.
  • Assists in GRYD presentations and events in the community as needed.
  • Proven ability to seek clarity, support, training, and conflict resolution direction ongoing.
APPLIED KNOWLEDGE:
  • Knowledge of gang environment.
  • Possesses in-depth knowledge of human service, health, court, and criminal justice systems' programs, regulations, and procedures.
  • Displays understanding of common issues facing young people and marginalized populations in the community where the program is located.
  • Diplomatic & professional communications skills.
  • Proven ability to utilize flexibility and openness to needed changes applied to best support population/program.
PROJECT MANAGEMENT/LEADERSHIP EXECUTION:
  • Work closely with Lead Case Manager to determine if FCM client is ready for exit or another cycle of services.
  • Provides case management of 60-150 FCM clients annually.
  • Responsible to administer the SET assessment tool.
  • Responsible for completing reports and imputing them into the GRYD ETO Database.
  • Performs outreach and marketing duties to recruit clients as needed.
  • Responsible to assist clients in obtaining Work Ready documents and/or making referrals to partners who could help.
  • Responsible to assist clients on all Tier 1 & 2 services.
  • Responsible to organize celebration activity meetings with clients and support system.
  • Other related duties as assigned by Lead Case Manager and President/GRYD Program Manager.
QUALIFICATIONS:
  • High School Diploma or equivalent
  • Bilingual a plus
  • Computer Literacy
  • Obtain a current California Driver’s License, clean DMV record & adequate auto insurance.
  • Must submit to a Department of Justice Criminal background check
