Actalent is Hiring a Medical Case Manager Near Hartford, CT

Seeking a caring and passionate mental health worker to join a community action agency serving our local communities in Hartford and Middlesex Counties. They're dedicated to helping people take steps toward healthy and economically stable futures. Job Description:

This individual will be responsible for Daily oversight of the SAMSHA HARP Program. Responsibilities include but are not limited to:

  • Oversees Peer Support Specialists.
  • Schedules, organizes, and chairs team meetings.
  • Works closely with Primary Care Provider and other medical providers for the program.
  • Works with Sub Recipient Evaluator and CRT Quality Assurance Staff.
  • Prepares timely and accurate reports to funding sources as requested by the Project Director.
  • Obtains all necessary training and certification in evidence-based interventions that are used in HARP.
  • Serves as a role model for Peer Support Specialists.
  • Assesses new evidence-based programs for program usage.
  • Stays up to date on research in HIV and substance use treatment to consider program improvements.
  • Networks on behalf of CRT and HARP with community agencies.
  • Arranges and maintains outreach schedules.
  • Works closely with Outreach Consultant to strengthen efforts to high-risk communities.

Schedule:

Monday 8:00am – 5:00pm (there is flexibility in terms of start time)

Qualifications:

  • Education: Bachelor’s degree in social work, health care administration, business administration or a closely related field but experience can be substituted for academic credentials
  • Minimum Years of Experience: Minimum of two years’ experience in the behavioral health field, particularly in the field of harm reduction (substance use and/or HIV)
  • Active Certification in good standing: Certified as a peer specialist or recovery coach, Community Health Worker, or Addiction Counselor or willing to obtain certification. [Preferred, not Required]
  • Demonstrate skills in: Excellent skills in the areas of interpersonal relationships, oral and written communication, public speaking, problem solving and decision-making. Excellent computer skills. Ability to work with staff and participants from a wide variety of backgrounds.
  • Knowledge of: JEDI (justice, equity, diversity, and inclusion) principles. Experience working with peer support specialists/recovery coaches and full understanding of their value to harm-reduction programs.
  • Bilingual: Needed
  • Active Driver License in good standing required upon hire and throughout employment.
