Prime Home Health and Pacific Hospice & Palliative Care is Hiring a LVN Licensed Vocational Nurse PRN - Hospice Near San Antonio, TX

Pacific Hospice and Palliative Care is seeking a compassionate and skilled LVN working on PRN basis to join our in-home hospice team. As an LVN in hospice care, you will play a vital role in providing comprehensive nursing care to terminally ill patients and their families in the comfort of their own homes throughout the San Antonio area. You will work closely with the interdisciplinary team to ensure the highest quality of care and support throughout the end-of-life journey. This is a rewarding opportunity to make a meaningful difference in the lives of patients and their loved ones during a challenging time.

Responsibilities:

  1. Patient Care: Provide direct patient care in accordance with the established plan of care and individualized patient needs, including administering medications, performing treatments, monitoring vital signs, and managing symptoms to promote comfort and alleviate pain.
  2. Care Coordination: Collaborate with the interdisciplinary team, including physicians, registered nurses, social workers, and chaplains, to develop and implement comprehensive care plans that address the physical, emotional, and spiritual needs of patients and their families.
  3. Assessments and Documentation: Conduct thorough assessments of patients' condition, document observations, and maintain accurate and timely medical records, including progress notes, medication administration records, and treatment plans.
  4. Education and Support: Educate patients and their families about their condition, treatment options, and end-of-life care, providing emotional support and guidance to help them cope with the challenges they may face.
  5. Collaborative Communication: Maintain open and effective communication with the interdisciplinary team, patients, families, and caregivers, ensuring the coordination of care, addressing concerns, and facilitating smooth transitions between healthcare settings.
  6. Emotional Support: Provide emotional support to patients and their families, promoting a caring and compassionate environment that respects individual beliefs, values, and cultural backgrounds.
  7. Safety and Compliance: Adhere to infection control protocols, maintain a safe environment for patients and staff, and comply with all relevant laws, regulations, and organizational policies and procedures.
  8. Professional Development: Stay updated with the latest developments in hospice care, attend educational programs, and participate in ongoing professional development activities to enhance knowledge and skills.

Qualifications:

  1. Valid LVN license issued by the State of Texas.
  2. Minimum of one year of experience required as an LVN in an in-home Hospice setting
  3. Proficiency in using Axxess EMR preferred, familiar with apps such as Slack and email. 
  4. Strong clinical skills and knowledge of nursing principles and practices.
  5. Excellent communication and interpersonal skills to interact effectively with patients, families, and the interdisciplinary team.
  6. Compassion, empathy, and sensitivity to the physical, emotional, and spiritual needs of patients and their families.
  7. Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic healthcare environment.
  8. Valid driver's license and reliable transportation for home visits. 
  9. Commitment to maintaining confidentiality and upholding ethical standards in patient care.
