Prime Home Health and Pacific Hospice & Palliative Care is Hiring a Chaplain - Hospice, Part-Time PRN Near San Antonio, TX

Pacific Hospice and Palliative Care is seeking a compassionate and empathetic individual to join our growing hospice team as a Chaplain on Part-Time PRN basis. As one of our hospice Chaplains, you will provide in-home spiritual support and guidance to patients and their families. Your role will be crucial in helping individuals navigate the emotional and spiritual challenges often associated with end-of-life care. You will create a peaceful and comforting environment, ensuring that patients and their families receive the necessary emotional and spiritual care during this sensitive time.

Responsibilities:

  1. Spiritual Guidance: Offer spiritual counseling, support, and guidance to hospice patients and their families in their homes, respecting their diverse beliefs, cultural backgrounds, and personal preferences.
  2. Emotional Support: Provide a compassionate presence and actively listen to patients and their families, addressing their emotional needs, fears, and concerns related to end-of-life experiences.
  3. Rituals and Ceremonies: Conduct religious ceremonies, rituals, or prayers according to patients' religious or spiritual beliefs, or assist in organizing such events if requested by patients or their families.
  4. Collaboration with Healthcare Team: Collaborate with the interdisciplinary hospice team, including physicians, nurses, social workers, and volunteers, to ensure holistic care and coordinate spiritual support for patients.
  5. Bereavement Support: Offer bereavement support to the family members and loved ones of patients, both during the patient's life and after their passing.
  6. Education and Training: Conduct educational sessions and provide training to hospice staff on spiritual and cultural considerations, end-of-life care, and ethical issues related to religious beliefs and practices.
  7. Community Outreach: Collaborate with local religious organizations and community groups to provide spiritual support services and foster partnerships that enhance the spiritual well-being of patients and families.
  8. Personal Development: Engage in continuous professional development to stay updated on best practices in chaplaincy, spiritual care, and end-of-life support.

Qualifications:

  1. You must possess a Bachelors degree with hospice experience, theology, church lay pastor experience or higher preferred.
  2. Experience: Previous experience as a chaplain in a healthcare setting, preferably in a hospice or palliative care environment is essential. 
  3. Knowledge of Axxess EMR or other EMR systems, computer knowledge of Microsoft Office programs and use of apps such as Slack.
  4. Organized documentation and meeting preparation: Maintain accurate, timely and confidential documentation of interactions, assessments, and spiritual care plans in compliance with organizational policies and regulations. Attend IDG meetings on a regular basis with Pacific Hospice team.
  5. Demonstrate a deep understanding and respect for diverse religious and spiritual beliefs, with the ability to provide appropriate care and support to individuals from various faith backgrounds or those with no religious affiliation.
  6. Exhibit exceptional interpersonal and communication skills, with the ability to connect empathetically with patients and families facing end-of-life challenges.
  7. Maintain high ethical standards, respecting confidentiality and professional boundaries when providing spiritual care.
  8. Possess the ability to cope with the emotional demands associated with working in a hospice environment.
  9. Work effectively as part of an interdisciplinary team, fostering collaborative relationships with colleagues and contributing to the overall care provided to patients.
