Position Overview: As an LMSW with Prime Home Health, you will have the opportunity to work closely with patients and their families, assessing their social and emotional needs. Your role will involve developing care plans, coordinating services, and providing essential support to promote overall well-being.

Key Responsibilities:

• Conduct comprehensive assessments of patients' social and emotional needs.
• Collaborate with interdisciplinary teams to develop and implement effective care plans.
• Provide counseling and support to patients and their families.
• Advocate for patients' rights and access to necessary resources.
• Maintain accurate and detailed records of patient interactions in Kinnser EHR. 

Qualifications:

• Licensed Master Social Worker (LMSW) in good standing in the State of Texas.
• Adult Home Health experience is preferred. 
• Must know Kinnser/Wellsky EHR and be able to use of other computer platforms such as Slack and BambooHR. 
• Strong communication and interpersonal skills.
• Compassionate and empathetic approach to patient care.
• Ability to work collaboratively within a team.
