Prime Home Health and Pacific Hospice & Palliative Care is Hiring a Registered Nurse RN - Hospice Near San Antonio, TX

We are seeking a dedicated and compassionate Registered Nurse to join our Pacific Hospice and Palliative Care on a W2 PRN basis. As an RN on our team, you will play a vital role in coordinating and providing holistic care to patients and their families during the end-of-life journey. Your expertise in palliative care, strong assessment skills, and ability to provide emotional support will contribute to enhancing the quality of life for our patients in their San Antonio area homes. The ideal candidate will possess excellent clinical knowledge, exceptional communication skills, and a deep commitment to providing compassionate care.

Responsibilities:

  1. Patient Care: Provide comprehensive and compassionate nursing care to patients in their homes or hospice facilities, ensuring their physical, emotional, and spiritual needs are met.
  2. Care Planning and Coordination: Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and their families, considering their unique needs, goals, and preferences.
  3. Assessment and Evaluation: Conduct thorough assessments of patients' physical and psychological status, including pain management, symptom control, and psychosocial support, ensuring ongoing evaluation of their condition.
  4. Medication Management: Administer medications, including pain management and symptom control medications, according to physician orders, and educate patients and families on medication management and potential side effects.
  5. Patient and Family Education: Provide education and support to patients and their families regarding the disease process, treatment options, pain management techniques, and end-of-life care, promoting understanding and involvement in the care plan.
  6. Emotional Support: Offer emotional support and counseling to patients and families, addressing their fears, concerns, and coping mechanisms during the end-of-life journey.
  7. Interdisciplinary Collaboration: Collaborate with the interdisciplinary team, including physicians, social workers, spiritual care providers, and volunteers, to ensure continuity of care, effective communication, and holistic support.
  8. Documentation and Compliance: Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements, hospice policies, and best practices.
  9. Bereavement Support: Provide bereavement support to families and caregivers following the loss of a loved one, offering resources, counseling, and referrals as needed.
  10. Continuous Education: Stay informed about current trends, best practices, and advancements in hospice and palliative care through participation in continuing education opportunities and professional development activities.

Qualifications:

  1. Registered Nurse: Current and unrestricted RN license in the state of practice.
  2. Proficient with computers: Axxess EHR or other hospice EHR; email; Slack and other platforms;
  3. Hospice Experience: Minimum of 2 years of clinical experience in hospice, palliative care, or a related field, demonstrating expertise in end-of-life care, pain management, and symptom control.
  4. Case Management Skills: Strong case management skills, including assessment, care planning, coordination, and documentation.
  5. Compassion and Empathy: Ability to provide compassionate and empathetic care to patients and families during the end-of-life journey, demonstrating sensitivity to their physical, emotional, and spiritual needs.
  6. Communication Skills: Excellent verbal and written communication skills, with the ability to effectively communicate with patients, families, and interdisciplinary team members.
  7. Critical Thinking: Strong critical thinking and problem-solving abilities to assess complex patient situations, make sound clinical judgments, and provide appropriate interventions.
  8. Time Management: Efficient time management skills to prioritize tasks, meet deadlines, and provide timely care while ensuring attention to detail and accuracy in documentation.
  9. Emotional Resilience: Ability to cope with emotional and challenging situations while maintaining professionalism and providing support to patients and families.
  10. Team Player: Demonstrated ability to work collaboratively in an interdisciplinary team, fostering a positive and supportive work environment.
  11. Regulatory Knowledge: Familiarity with hospice regulations, including Medicare Conditions of Participation and Joint Commission standards.
