Prime Home Health and Pacific Hospice & Palliative Care is Hiring a Hospice Community Liaison - Marketer, Full-Time Near San Antonio, TX

As an empathetic Hospice Community Liaison, you will play a crucial role in fostering relationships with physicians and healthcare professionals to educate them and their patients about hospice care and the services provided by Pacific Hospice. You will serve as the primary point of contact, promoting the understanding and utilization of hospice services, while demonstrating empathy, compassion, and expertise in end-of-life care. Your ability to build rapport, effectively communicate, and provide comprehensive information will contribute to improving the quality of life for patients and their families.

Responsibilities:

  1. Relationship Building: Cultivate and maintain strong relationships with physicians and healthcare professionals, including nurses, social workers, and case managers, to ensure they are aware of and knowledgeable about Pacific Hospice's services.
  2. Education and Training: Conduct regular meetings, presentations, and educational sessions with healthcare professionals to educate them about the philosophy, benefits, and scope of hospice care, and specifically highlight the services provided by Pacific Hospice.
  3. Patient Advocacy: Collaborate with healthcare professionals to advocate for patients who may benefit from hospice care, ensuring their understanding of the referral process and the criteria for hospice eligibility.
  4. Resource Management: Maintain an up-to-date understanding of hospice regulations, policies, and industry trends to effectively address questions, concerns, and challenges faced by healthcare professionals and provide accurate information and resources.
  5. Communication: Serve as a liaison between healthcare professionals, patients, and Pacific Hospice's interdisciplinary team, ensuring effective communication, coordination, and collaboration to meet the needs and preferences of patients and their families.
  6. Relationship Expansion: Actively identify and engage with new healthcare professionals, clinics, hospitals, and community organizations to expand partnerships and referral networks, contributing to the growth of Pacific Hospice's services.
  7. Documentation and Reporting: Maintain accurate records of interactions, referrals, and educational activities, ensuring timely and detailed reporting to the management team.

Qualifications:

  • Bachelor's degree in a healthcare-related field (e.g., nursing, social work, healthcare administration) preferred.
  • Previous experience working in a hospice or palliative care setting, or strong knowledge and understanding of end-of-life care and services.
  • Excellent interpersonal and communication skills, with the ability to empathize, actively listen, and effectively convey complex information to diverse audiences.
  • Strong networking and relationship-building abilities, with a proven track record of successfully engaging healthcare professionals.
  • Knowledge of hospice regulations, industry standards, and current healthcare practices related to end-of-life care.
  • Highly organized, detail-oriented, and able to manage multiple priorities effectively.
  • Proficient in using productivity tools, such as Microsoft Office Suite and customer relationship management (CRM) software.
  • Compassionate, caring, and able to maintain emotional resilience while working in sensitive end-of-life situations.
  • Valid driver's license and ability to travel within the assigned territory as needed.
