As a Hospice Nurse, you will be responsible for overseeing the quality, expert care of your assigned hospice patients. This opportunity offers $34- $43 per hour based on experience! Obtain patient's history on admission
Establish and implement an individualized plan of care for each patient
Perform initial and on-going patient assessments
In collaboration with other IDT members, evaluate and revise the interdisciplinary care plan on on-going basis
Assess the need for patient and family education and provide education as needed
Obtain appropriate physician orders to achieve symptom management
Provides safe and appropriate skilled nursing care in accordance with protocols, policies, procedures and physician orders
Provide supervision of all CHHA
Manage and administer medications according to standards
Document in EMR and/or paper chart in the IPU in an accurate, concise and timely manner per standards
Perform effectively in emergency situations
Follow OSHA and agency regulations in the use of standard precautions, the management of controlled substances and biological
Provide or facilitates the provision of emotional and spiritual support to patients and their families
What You'll Bring to Our Team:
Minimum of one year of related nursing experience.
NJ RN license required; BSN preferred
Bilingual is a plus!
