Medical Director

WHO WE ARE & PURPOSE OF POSITION

For over 50 years, Covenant House New York (CHNY) has been a leader in providing a full continuum of shelter, transitional and permanent housing and wrap-around support services for vulnerable homeless, runaway and exploited youth. As the largest provider of runaway and homeless youth services in New York City, CHNY serves more than 1,500 youth and young adults ages 16-24 annually. Our doors are open to all young people who need help, regardless of their race, religion, sexual orientation, gender identity or expression. We provide shelter, housing, education, employment, health, mental health and other services using harm reduction and trauma-informed principles. We also advocate for systemic change through public policy while promoting awareness of critical issues related to youth homelessness. Covenant House New York’s goal is to help each young person find safety and refuge from the dangers of living on the streets and to empower youth now and into the future. We do this through compassion, unconditional love, and absolute respect.

The Medical Director of CHNY’s Health & Wellness Center is the lead clinician, in charge of all aspects of medical patient care services provided in our Federally Qualified Health Center (FQHC). Our Health & Wellness Center provides services to youth in CHNY programs as well as members of the community. The Medical Director will monitor clinical performance to ensure medical services and operations comply with all applicable regulatory and licensing agencies and government grants. As a member of the Leadership team, the Medical Director is responsible for reviewing and developing clinical policies and protocols, compliance measures, provider workflow planning, and assisting with provider scheduling. The Medical Director oversees the Health & Wellness Center’s Quality and Risk Management (QRM) Plan. The position serves as coach/mentor/trainer to providers, promoting a clinical vision and giving guidance in best practices, troubleshooting medical services and operations issues in priority of urgency according to CHNY policy and FQHC standards. The Medical Director works in close collaboration with other departments across CHNY, particularly the residential and mental health teams. Further, the Medical Director cultivates relationships with other healthcare entities for partnerships and referrals for our patients.

As a licensed Physician, the Medical Director will also provide direct patient services in addition to their duties in administration. The Medical Director will provide quality primary care services, meeting the diverse health and social needs of our patients. The Medical Director will perform complete examinations of patients and screen all patients for health maintenance needs. The Medical Director will document clinical observations, adhere to industry standards and conduct chart reviews to ensure compliance. This role is in-person with some flexibility for remote administrative activities.

DUTIES AND RESPONSIBILITIES

 (Responsibilities to include but not limited to the following)

GENERAL

  • Provides leadership, vision, and expertise in the areas of clinical practice, service delivery, and regulatory compliance for the Health & Wellness Center.
  • Functions as a member of the Health & Wellness Center’s management team and reports on the Center’s progress to meet patient needs.
  • Ensures compliance with legal, regulatory and other requirements applicable to FQHCs and New York State Article 28 Clinics.
  • Follows and adheres to the Medical Standard of Care for New York City and assures that the clinic follows the guidance of Evidence-Based Medicine in accordance with both the American Academy of Family Practice and the American Academy of Pediatrics.
  • Follows guidelines established by OSHA, the New York State Department of Health, the NYC Department of Health and Mental Hygiene, CLIA and the CDC.
  • Creates and implements best practice programs, while maintaining a consistent level of quality of care offered to patients while embodying and upholding the mission of Covenant House New York.
  • Performs other duties as assigned and needed.

CLINICAL

  • Provide direct patient care, including preventive care, primary diagnosis and treatment as clinically appropriate.
  • Maintains strict patient confidentiality, including compliance with HIPAA, relevant New York State confidentiality laws and regulations, and CHNY Child and Youth Protection Policies.
  • Maintains timely documentation in medical records in accordance with established clinic protocols.
  • Leads clinical practice quality improvement activities.
  • Provides on-call coverage.
  • Coordinates referral of patients with serious medical needs with surrounding specialists in the New York City area.
  • Oversees reviewing and/or monitoring of all laboratory, medication/pharmacy, x-ray, and ancillary activities performed by staff assigned to these services.
  • Maintain admitting privileges at a minimum of one local New York City Hospital.

SUPERVISORY DUTIES

  • Supervises medical staff, including selecting, orienting, and training, coaching, mentoring, disciplining, and performing timely performance reviews.
  • Maintains collaborative relationships with midlevel providers.
  • Responsible for directing all providers in the practice of quality care according to standards and benchmarks set by the organization, licensing boards and funding agencies.
  • Assists in the achievement of productivity standards and provides leadership within the Health Center team to achieve those standards.
  • Oversees medical chart reviews/chart audits/quality audits, including the peer review process
  • Develops and oversees (in conjunction with the Leadership Team) recruitment and retention program for clinical providers.
  • Oversees privileging of clinical providers.
  • Handles complaints involving staff under their supervision in coordination with the People & Culture team.
  • Acts as the primary supervising physician.
  • Provides clinical consultation and recommendations to Health & Wellness Center provider staff.
  • Originate and lead monthly providers’ meetings.

ADMINISTRATIVE

  • In partnership with the CPO and Compliance team, develops and monitors appropriate standards of care, clinical policies and procedures, quality improvement indicators, and patient care protocols through evidenced-based programs and processes.
  • Serves as an active member of the Leadership Team, preparing for Board meetings and reporting to the Board of Directors and other administrative meetings as required.
  • Represents the CHNY Health & Wellness Center to other agencies, medical groups, institutions, and the community as necessary.
  • Develops and manages referral partnerships to provide medical care for patients.
  • Oversees the clinical component of the quality improvement program. In partnership with the Leadership Team, develops and implements improvement and monitoring indicators.
  • Performs due diligence in matters related to clinical practice in full compliance of the medical licensing board.
  • Provides additional assistance with other tasks assigned by the CPO and/or Board of Directors.
  • Ensures that the Quality Assurance/Quality Improvement process results in improved coordination of care and services for patients, thereby achieving established targets and goals.
  • Ensures Medical Rounds and daily huddles are performed on a daily basis.
  • Supervises and oversees patient flow in collaboration with the Health Center Administrator.
  • Develops and implements policies, procedures and best practices for all clinical service areas.
  • Manages the department's budget in coordination with the Health Center Administrator.
  • Ensure timely submission of all required statistical analysis of the center's performance including reports to funding agencies, Board, and Chief Program Officer.
  • Oversees risk management efforts.
  • Chair the quarterly infection control committee.
  • Ensures the Health & Wellness Center operates in a manner that is ready for any audit.
  • Review and approve the annual HRSA Uniform Data Systems (UDS) Report.

Qualifications


KNOWLEDGE, SKILLS, & ABILITIES

Education & Experience

  • Board Certification/eligible in a medical field relevant to primary care.
  • Physician currently licensed and registered in New York State.
  • Preferably, direct applicable experience in a physician leadership/physician manager role, including supervisory skills in a mentoring/coaching capacity in a FQHC, community-based ambulatory care setting, or equivalent.
  • Preference given for experience and commitment to working with homeless youth
  • Must have current CPR certification and DEA license.
  • Must have hospital admitting privileges at one New York City hospital

Knowledge/Skills

  • Strong public health services philosophy and leadership skills.
  • Demonstrates a high level of skill with interpersonal relationships; excellent written and verbal communication required.
  • Excellent critical thinking, deductive reasoning and decision making skills, as well as superior patient/customer service, ethics and bedside manners; strong organizational and reporting skills are imperative.
  • Should also have exceptional research and writing skills, and must constantly seek out new procedures to assist the Health Center in running as efficiently as possible.
  • Overall flexibility in work schedule required, including evenings as needed.
  • Ability to efficiently and effectively manage multiple clinical tasks, and collaborate as part of a multi-disciplinary team.
  • Knowledge of Current Procedural terminology coding (ICD-10 codes) and Healthcare Effectiveness Data and Information Set (HEDIS measures ).

The salary range $225k -$231,737k.

Compensation

Covenant House New York offers a rich package of benefits including but not limited to full medical, vision, dental, life and short and long-term disability insurance and other fringe benefits. Eligibility for medical benefits begins on the date of hire. PTO includes 3 weeks of vacation in the first year, 10 holidays, 13 sick days, 5 personal days and birthday days off. Benefits include a 403b with employer contribution after 12 months. There are also multiple additional optional benefits (including additional life, disability, accident and cancer insurance) available at significantly reduced rates. "CH is a vaccinated employer.
