Responsibilities:

• Roll up in style to perform clinical services in a custom health van.
• Assess patients with a known history of substance abuse disorders.
• Diagnose and treat ailments with the city skyline as your backdrop.
• Collaborate with our team of healthcare explorers.
• Educate and empower communities with your medical wisdom.
• Keep your mobile clinic stocked with a treasure trove of health goodies.
• Maintain medical records while on the move.
• Coordinate services by making referrals to appropriate medical and behavioral health facilities and following up on such referrals.
• Additionally, coordinate with Medical Doctors and clinic staff for tele-practice services.

Qualifications:

• NYS License to practice as a Registered Professional Nurse and certification by the NYSED in medicine to practice as a Nurse Practitioner.
• Must be eligible and maintain eligibility for full and unconditional participation in the Medicaid and Medicare programs
• DEA
• NYS Prescriptive Rights
• Behavioral Health background preferred
• Must have computer skills
• A spirit of adventure and a love for exploring diverse communities.
