Job description

Position Summary:

As a Street Outreach Peer Specialist, you will use your personal lived experience to connect with individuals facing substance use and mental health challenges. Reporting to the Program Director, you will provide essential support by assisting with decision-making, sharing coping skills, and linking individuals to clinical treatment, harm reduction, recovery services, and other crucial supports. Your role will involve outreach in local communities and providing transportation to ensure individuals can access the services they need in Nassau and Suffolk Counties.

  • Lived Experience: Utilize your own experiences with mental health and/or substance use services to connect and empathize with individuals.
  • Engagement: Conduct field and phone outreach to locate and engage individuals who require support.
  • Peer Recovery Support: Share your life experiences to assist clients in developing recovery plans and connecting them with community-based recovery programs.
  • Education and Guidance: Educate individuals about available program services, benefits, and self-help techniques.
  • Treatment Support: Assist individuals in clarifying treatment goals and developing personalized treatment plans.
  • Crisis Intervention: Support program participants in pre-crisis or crisis situations.
  • Recovery Skills Training: Teach recovery skills based on shared life experiences.
  • Resource Sharing: Inform participants about available benefits, entitlements, and other support services.
  • Transition Assistance: Help individuals transition between different levels of care as needed.
  • Community Linkages: Provide links to community-based treatment and recovery support services for individuals and their families.
  • Symptom Management: Educate individuals on symptom management and coping skills.
  • Transportation and Referrals: Provide transportation and warm handoffs to treatment services when required. Educate and refer individuals for MAT services and assist in applying for benefits or housing.
