We are seeking a Full Time Nurse Practitioner for a private Cardiology & Internal Medicine outpatient office in Brooklyn, NY.
We are willing to train. New grads welcomed.
Salary is $120K-$140K a year depending on experience plus full benefits.
Work Schedule:
· Your schedule will range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m.
· An average workweek totaling approximately 40 hours.
Responsibilities:
· Conduct comprehensive health assessments and screenings for individuals and families in the community.
-Provide Stress Tests
-Order Chest X-Rays and Diagnostics
-Treat Peripheral Arterial Disease (PAD)
-Manage patient comorbidities
-Treat and manage patients' hypertension, diabetes, high cholesterol
-Medication Management for patients with CHF and Atherosclerosis
· Diagnose and manage acute and chronic medical conditions, including preventive care and health promotion.
· Collaborate with community organizations and stakeholders to develop and implement health education programs.
· Provide culturally sensitive care and advocate for patients' rights and access to healthcare services.
· Coordinate referrals and follow-up care to ensure continuity of care for patients.
· Maintain detailed and accurate medical records in compliance with regulatory standards.
· Participate in community outreach events and health fairs to raise awareness and provide health screenings.
· Stay updated with community health trends, evidence-based practices, and relevant policies.
Requirements
Requirements:
· Current certification as a Nurse Practitioner; FNP, ANP, or AGNP.
· Active New York State APRN License
· Master's degree in nursing from an accredited program.
· Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
