Position Overview:

The Vice President of Clinical Transformation is responsible for leading initiatives that enhance population health outcomes, alternative payment model performance, foster strong payor relationships, and leverage technology and data analytics to improve quality outcomes and operational efficiencies. This strategic role involves overseeing key areas such as payor relations, payor and alternative payment contracting, care coordination, population health, system redesign and performance improvement, data analytics, and information systems management. The role collaborates with internal and external stakeholders to align these initiatives with the Neighborhood Family Practice's mission.

Key Deliverables and Responsibilities:

  • Develop and execute strategies to improve population health and value-based care performance. Build and maintain strong relationships with payors and third-party partners regarding contracts and collaborations that enhance patient care and organizational sustainability.
  • Lead the Data Analytics team to harness insights from healthcare data, driving improvements in clinical outcomes, patient satisfaction, and operational efficiency.
  • Lead Population Health team in its efforts to coordinate care via outreach and care coordination.
  • Coordinate organizational efforts with Performance Improvement in order to achieve goals related to value based pay and improved clinical quality metrics.
  • Advocate for and implement advanced analytics and data visualization tools to support decision-making and strategic planning.
  • Manage organizational strategies and implementation for data platforms within the EHR (Epic) and external (Azara, Aledade, etc).
  • Guide the integration of value-based care principles and key performance indicators into clinical practices, ensuring alignment with quality standards and payor requirements.
  • Manage cross-functional projects that involve clinical teams, IT, finance, and external partners to achieve targeted outcomes.
  • Oversee information systems implementations that support clinical and operational goals.
  • Other duties as assigned


Qualifications:

  • Advanced degree in Healthcare Administration, Business Administration, Public Health, or a related field.
  • Proven experience in value-based care models, payor relations, and data analytics within a healthcare setting.
  • Strong understanding of healthcare financial models, reimbursement mechanisms, and regulatory compliance.
  • Excellent leadership, communication, and interpersonal skills.
  • Demonstrated ability to lead change and innovate in complex healthcare environments.
  • Proficiency in data analysis tools and electronic health records systems.

  • Office work environment with the need for adaptability to rapidly changing priorities.
  • Ability to travel as needed for meetings with payors, partners, and stakeholders.
  • Proficiency in electronic communication and collaboration tools.
