JOB CONTENT

  1. Position Summary:

(Overall purpose of this position)

The HIV prevention coordinator will ensure patient navigation measures are implemented which support patient care and expedite referrals to internal and external resources for medical and non-medical needs. This position will link patients to screening procedures for HIV and other sexually transmitted infections. The HIV Prevention Nurse and HIV Prevention Coordinator will collaborate to provide various trainings and educational offering for NFP staff and community.

  • Provide case management for individuals at risk of acquiring HIV.
  • Will organize and conduct HIV prevention services, testing, reporting and outreach.

Collaborate with the HIV prevention nurse, NFP providers and other staff on HIV

prevention measures

  • Present safer sex outreach/prevention education and risk reduction Maintains quality of testing tracking and reporting results by following department procedures and testing schedule; recording results in the quality-control log; identifying and reporting needed changes.
  • Keeps current with standards of care regarding HIV prevention and testing.
  • Gather and report HIV Data and analysis of program results on monthly basis.
  • Coordinates work with community engagement team on outreach and engagement to build community relationships within at-risk communities and deliver education on NFP's HIV prevention services and comprehensive HIV program.
  • Works closely with NFP's Integrated HIV Primary Care Team to coordinate linkage to care and medical case management
  • Remain current on local resources to address social determinants of health
  • Ensure that all applicable consent forms are signed by patients and uploaded to patients' records, and always maintain patient confidentiality
  • Keeps current with pertinent laws and regulations regarding HIV testing and counseling as well as professional principles and practices in the fields of health care and health data analysis
  1. Qualifications:

  • Bachelor's Degree in Social Work must be a Licensed Social Worker with two years of

related experience preferred.

  • Strong Organizational and Project Management Experience
  • EPIC or similar EHR system experience
  • Knowledge of and comfortable with Microsoft programs (Word, Excel, Teams) for

documenting and presenting work results

  • Knowledge of community and familiarity with HIV prevention programs and resources

preferred

  • Core values consistent with a patient-and family-centered approach to care.
  • Demonstrates professional, appropriate, effective, and tactful communication skills,

including written, verbal and nonverbal.

  • Demonstrates a positive attitude and respectful, professional customer service.
  • Acknowledges patient's rights on confidentiality issues, always maintains patient

confidentiality, and follows HIPAA guidelines and regulations.

  • Proactively acts as patient advocate, responding with empathy and respect to resolve

patient and family concerns, and recognizes opportunities for improvement to

meeting patient concerns.

  • Actively pursues training on education on providing quality care and enhancing

professional skills.

  • Adheres to professional code of ethics, including maintaining professional boundaries with patients and their families.

  • Ability to travel amongst organizational sites and within the community as needed.
  • Availability to work flexible schedule including evenings and weekends: local travel

and some out-of-town travel required.

  • Must have current Driver's License with insurance.
