NEIGHBORHOOD FAMILY PRACTICE is Hiring a Licensed Practical Nurse, Clinical Support Near Cleveland, OH

1. Position Summary:

(Overall purpose of this position)

Under the direction of a provider or registered nurse:

  • Support providers in the examination and treatment of patients, including the preparation of exam rooms and instrumentation, operation of medical equipment, administer medications and injections, etc. related to patient care requirements.
  • Provide nursing care in a community health ambulatory care setting including health promotion, patient and family support, health maintenance, disease management, coordination of care and maintaining continuity of care.
  • As a LPN, Clinical Support, this position is accountable for being an active member of the patient centered medical home team.

2. Essential duties and responsibilities which must be performed in order to carry out the position purpose summarized above:

A. Train new MA/LPN staff on workflow and NFP policies and procedures.

B. Ensure all temperature sensitive deliveries are processed in a timely manner.

C. Assist with downloading of monthly data loggers and files in appropriate Teams folder.

D. Maintain weekly and monthly quality controls.

E. Conduct daily, weekly and monthly autoclave cleanings and weekly spore testing.

F. Conduct monthly cleaning of microscopes.

G. Perform weekly stock medication and private/VFC vaccine counts.

H. Assist with appointment scheduling and outreach calls.

I. Establish reason for visit and document patient description of chief complaint. Reviews medication list, allergies, pharmacy and immunizations. Document all EPIC visit encounter fields and initiates visit note for provider.

J. Participate in quality improvement and workflow enhancements that support patient care.

K. Take and document pertinent vital signs including blood pressure, pulse, respirations, pulse oximetry, height, weight, head circumference (if applicable) and pain level.

L. Assure all needed POCT are completed and documented in EMR prior to provider visit. All back -office test results MUST be documented prior to end of day.

M. Pre chart scrubbing, needs to be completed prior to patient’s arrival for appointment. Pre charting to include-review of Health Maintenance, review of patient problem list/diagnosis and pending of orders for due immunizations, labs, referrals and screening questions that may be due.

N. Knowledge/set up of appropriate instrument trays to be completed prior to patient appointment.

O. Follow-up on provider post exam requests; dress wounds, perform ear irrigations, recheck blood pressure etc.

P. Administer injectables deemed appropriate per provider order and or immunizations.

a. Document 15 min wait for appropriate vaccines and provide VIS sheets for all immunizations. All medications and vaccines MUST be documented prior to end of shift.

Q. Maintain patient flow for providers, filling rooms per designated room allotment.

R. Assist with daily refrigeration temperature monitoring and vaccine supply storage units.

S. Properly disposes/packages sharps, biohazards and expired/wasted medications in appropriate containers.

T. Completes established daily opening and closing procedures as assigned.

U. Clean and stock rooms per established requirements.

V. Assist with ordering supplies, immunizations and provide assistance as directed or needed by Site Manager, Department Manager or Lead MA.

W. Assist with LARC count and rotation of medical supplies in the clinic

X. Assist with monthly work orders to include the following-AED check and battery check on clinic scales

Y. Serve as a member of a Patient Centered Medical Home team.

Z. Position may require travel to other NFP sites, with or without prior notice to assist with coverage and call offs.

AA. Position will be required to work some evening hours. Weekend hours may be required if/when the organization deems it necessary to increase access to patient care.

  • Licensed Practical Nurse in the State of Ohio.

  • Must have current BLS certification.

  • Knowledge of injection levels and procedures (children and adults).

  • Knowledge of universal precautions, bloodborne pathogens and OSHA guidelines

  • Demonstrated understanding of scope of practice and clinical nursing skills.

  • Demonstrated critical thinking skills and ability to work independently.

  • Demonstrated commitment to continuing professional development through participation in continuing education programs.

  • Demonstrated computer proficiency, EMR.

  • Demonstrated interpersonal skills, cultural competency and understanding of underserved populations.

  • Demonstrated team membership skills.

  • Previous experience that is
    necessary background to qualify for this position:

  • Moderate level of broad, general nursing experience.
