POSITIONSUMMARY
Responsible for carryingout the daily habilitation goals and objectives for qualified HCB Waiverclients in community settings as assigned.
This position reports directly tothe Senior and Disabilities Services (SDS) Director.
PRINCIPALDUTIES AND RESPONSIBILITIES
1.Treatsindividuals with respect and upholds their human rights.
2.Providesdirect client support during any scheduled involvement in community settingswhile encouraging maximum independence, safety, and healthy habits while out inthe community.
3.Providesindividualized skills training related set Plan of Care goals and objectivesthat fosters successful participation in community settings.
4.Participatesin the design of and on-going implementation of individualized service plansfor all qualified clients receiving Day Habilitation Services while maximizingthe use of community resources.
5.Meetsregularly with clients Care Coordinator for client goal and objective progress.
6.Maintainsgood communication with clients’ guardians of care providers, ensuring anyimportant need to know information is shared.
7.Submitsprogress reports on a regular basis.
8.Assistsin any reports pertaining to Day Habilitation services.
9.Assistsin the arrangements for individual transportation and times to communitysetting events pertaining to individual goals and objectives.
10.Completesall required documentation and assists with any needed amendments to individualservice plans.
11.Assistswith internal and external audits and evaluation needs.
12.Complieswith Maniilaq SDS program standards, and Medicaid requirements.
13.Maintainsthe required State licensing, continuing education hours annually.
14.Maintainsgood driving record for continued Maniilaq insurance coverage.
15.Maintainsthe ability to be flexible with scheduling Day Habilitation service coverage,including possible special community events.
16.MaintainsFirst Aid / CPR certification.
MINIMUMREQUIREMENTS
Highschool diploma or equivalent preferred. The desire to work with people withdevelopmental disabilities. Ability to lift up to 75 pounds and assist clientswith mobility issues.
Must obtain CPR / First Aide certification within 6 monthsof hire. Ability to accurately and efficiently use standard office computerequipment and software, faxes, copiers, telephones, and 2-way radios.
AlaskaDriver’s License required with a good enough record to be insured to driveManiilaq vehicles.
