Transitions Care - Central Illinois is Hiring a PRN / Chaplain Near Springfield, IL

Description

Transitions Hospice is seeking a Chaplain to join our family! This position is responsible for the provision of non-denominational spiritual care services to patients/families/caregivers, either directly or through coordination of care with other spiritual counselors.

We are seeking candidates who shares our philosophy: to empower our patients and families by providing the tools and support necessary to embrace life. We don’t focus on the finality of death; we focus on the practical side of living. Our job is to assist our patients and families through the physical, emotional, and spiritual challenges of life’s final transition. Social working experience is a plus. Local Travel Necessary

Qualifications

  • Graduate of accredited seminary or school of theology (B.D., M.Div. or equivalent theological degree), or degree in counseling, or equivalent experience.
  • Experience working with death and dying, individual/family/caregiver.
  • Comfort in an interfaith setting.
  • Ability to network with community clergy and congregations.
  • Hospice/home health experience preferred.
  • Valid driver's license with maintenance of good driving record.
  • Reliable transportation with valid auto insurance.

Requirements

  • Assess spiritual needs of patients/families/caregivers in a timely manner, and provide consultation to patients, families, caregivers and team members in a non-denominational manner or as requested by patients/families/caregivers and communicate to the Patient Care Manager and staff.
  • Contact patient's/family's preferred spiritual advisor to ensure ongoing involvement and to invite participation in interdisciplinary care planning when requested by team member or patient family member.
  • Contact clergy or spiritual counselors to assist in spiritual care of patients and family members.
  • Develop and document plan for spiritual care, in conjunction with the interdisciplinary team when requested by team member or patient family member.
  • Evaluate and revise plan for spiritual care of patients/families/caregivers with the interdisciplinary team on an ongoing basis.
  • Maintain records of spiritual care services utilization and related activities for quality assurance, program development and policy and procedure review and revision.
  • Maintain working list of local community chaplains and spiritual counselors.
  • Document direct services and ongoing communication with community chaplains and spiritual counselors.
  • Provide consultation, education, and support to the interdisciplinary team on spiritual care.
  • Conduct Pre-Bereavement contacts with the patient and family as assigned in the Hospice Plan of Care.
  • Conducts Bereavement assessments and Bereavement contacts as assigned.
  • Establish networks with faith communities throughout the service area for the purposes of teaching about the hospice philosophy and enlisting aid in providing spiritual support.
  • Provide for funeral or memorial services for patients as requested.
  • Conduct memorial services in nursing homes as requested by Transitions Hospice management.
  • Conduct or offer not less than one memorial service per year for family members of Odyssey's bereavement program.
  • Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers.
  • Coordinate travel time for visits and manage a caseload to deliver care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies.
  • Participate in evening/weekend on-call as required, conducting on-call services in a clinically competent and responsive manner.
