Transitions is seeking a Patient Care Manager with hospice and/or health care experience to join our growing family! The ideal candidate will assists the Administrator/Director of Nursing with adherence to the policy and procedure manuals that establish and support the highest possible quality of care to patients and families. Local Travel is Necessary.

The Patient Care Manager (PCM) is a member of the leadership team that works collaboratively with the Administrator/Director of Nursing with all patient care activities to ensure the delivery of safe and efficient patient care which adheres to relevant standards of accepted nursing, regulatory, medical practice, and payer source criteria. The PCM works to offer support and education to team members that is consistent with the identified needs based on a review of the documentation, compliance audits and reported areas of weakness.
