What You Will Need

  • Current CPR certification or must obtain within 6 months of employment.
  • ASCP certification or equivalent is preferred.
  • If responsible for collecting Nursing Homes, must possess a valid Illinois class “D” driver’s license and satisfy the annual Drivers Motor Vehicle verification review.

I. General SummaryReports to the Laboratory Director with accountability to the Lab Support Services Supervisor, Lab Support Services Manager and the Chief Phlebotomist. Performs venipuncture, finger sticks, heel sticks and other specimen collection methods according to departmental and Northwest Community Hospital (NCH) policies and procedures for inpatients, hospital outpatients, client sites and/or nursing home clients.
Ii. Representative FunctionsPhlebotomist II (0489) – Draw sites, NCH and client based Demonstrates the knowledge and skills necessary to assess and treat all age groups of patients from neonatal to geriatric; implements treatment based on the developmental stages of the patient. Performs venipuncture, finger sticks, heel sticks, POC testing (if offered at the draw site location), and other specimen collection according to policy and procedure. Instructs the patient in the collection of urine specimens and other patient collected specimens. Identifies correct patient before collecting specimen; labels all specimens accurately before leaving patient. Completes phlebotomy procedure and specimen process for all timed specimen pick-ups, STATs (based on lab scope of services by location) and special lab orders within established time limits.
Registers the patient in the relevant and appropriate electronic systems, inputs outpatient laboratory orders into the electronic system at the point of service according to compliance regulations; researches and resolves physician orders needing clarification by contacting the physician or office as needed. Identifies Medicare patients requesting services potentially not covered by the Medicare Plan according to compliance regulations; completes an Advance Beneficiary Notice (ABN) when indicated for non-covered services prior to providing services.
Processes and documents verbal and written lab orders and standing orders in accordance with regulatory standards and hospital policy.
Prepares and packages specimens for transport as required using manifests and transport logs. Maintains records such as logs, workload records, and requisitions in a neat and organized manner.
Performs laboratory procedures according to procedure manuals, evaluates test results, including normal and abnormal results, which require notification, documentation and or verification, and communicates issues to manager.
Maintains operational level of equipment, cleans when needed, performs maintenance procedures and basic troubleshooting, and reports all malfunctioning equipment to manager.
Performs test procedures on quality control specimens, documents the results, takes corrective action when indicated and notifies manager when results are out of control; participates in quality assurance programs.
Monitors supply quantities at the draw site and/or physician office site when needed and notifies designated contacts of inventory needs.
Serves as a resource to other Phlebotomists and team members and assists in training.
Adheres to scheduling requirements with variable locations as determined by patient, staffing and budgeting fluctuations.
Practices appropriate Standard Precautions as described in Exposure Control Plan. Adheres to all other departmental and hospital safety standards in order to maintain a safe patient care and working environment.
Bilingual (English/Spanish) required based on assignment.
Adheres to all Northwest Community Hospital standards, policies and procedures.
V. Physical RequirementsModerate physical effort. Requires continuous standing, bending or walking. May require frequent lifting or moving of average weight material, or occasionally lifting or moving heavy weight material.
The flow of work and character duties involves normal mental and visual attention along with manual coordination much or all of the time.
Vi. Working ConditionsConditions are acceptable. At least one disagreeable element or hazard is frequently present in the environment, i.e. exposure to infectious disease, while several disagreeable elements or hazards are occasionally present.
