Social Services Assistant

Starting Salary Range: $17.1735 -18.7084/hour

The following position is available at the McLean County Nursing Home located in Normal, IL.

Position Summary

Under general supervision, this position will develop and maintain relationships with our residents and assist them with a variety of daily tasks and concerns.

Responsibilities

  • Assist with making appointments for needed resident services
  • Call family members for resident needs
  • Stay current with resident conditions, events of the day, concerns etc. through use of in-house communication
  • Build relationships between residents and staff and help staff to understand and support resident’s individual needs
  • Assist residents in utilizing individual and group activities to their best advantage
  • Assist with room changes/send memos
  • Admission paperwork, tours of facility and referrals when needed
  • Assist with discharge planning
  • Complete MDS assignment sections, care plan and write progress notes
  • Shop for requested resident items and pick up orders
  • Attend scheduled meetings
  • Any additional duties as requested by the Social Services Director or Nursing Home Administrator

Knowledge, Skills, and Abilities

  • Good attention to detail
  • Ability to communicate by phone with a variety of different parties
  • Ability to remember multiple messages, information and assignments given to self and others
  • Ability to read and write in English
  • Ability to remain calm under stress
  • Ability to concentrate on fine details with frequent interruptions necessary to perform a variety of functions
  • Ability to communicate with a wide range of personalities

Required Qualifications

  • High school diploma
  • Valid driver’s license

Preferred Qualifications

  • Some college coursework in human services related field
  • Prior long-term care experience and/or prior work in a social service environment

Physical Requirements

  • Occasionally squats, reaches above shoulder, crouches and kneels
  • Occasionally lifts 11-50 pounds and carries 11-24 pounds
  • Frequently pushes/pulls, lifts and carries up to 10 pounds
  • Walks or stands 1-4 hours of the 8-hour workday

In 1974 the citizens of the County decided to continue the tradition of providing care to the elderly when they passed a referendum to build the current Nursing Home, at its current location.
