Responsibilities
- Provide personalized in-home care tailored to each client's needs, including assistance with daily activities such as bathing, dressing, and mobility
- Monitor and document clients’ health status, reporting any changes or concerns to healthcare professionals promptly
- Administer medication reminders or assistance as prescribed by healthcare providers
- Support clients with rehabilitation exercises or therapies as directed by medical teams
- Assist with light housekeeping tasks related to client care to maintain a safe and clean environment
- Coordinate with family members and healthcare teams to ensure comprehensive care plans are followed effectively
- Maintain confidentiality and adhere to all healthcare regulations and safety protocols
Requirements
- Proven experience in healthcare settings such as Level I Trauma Centers, Level II Trauma Centers, PICU, Geriatrics, Medical-surgical units, or outpatient clinics
- Prior experience in hospital medicine, emergency medicine, post-acute care, or sub-acute care environments is preferred
- Strong understanding of patient safety protocols and infection control practices
- Excellent communication skills and compassionate nature
- Ability to work independently while managing multiple clients’ needs effectively
- Certification in CPR/First Aid is preferred but not mandatory; willingness to obtain certification is a plus
- Valid driver’s license and reliable transportation are advantageous for travel between client locations
