Responsibilities
- Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by checking vital signs and observing for any changes in condition
- Administer medications accurately following care plans and HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Support patients with memory care, dementia, Alzheimer's, or other cognitive impairments using specialized techniques
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning and laundry
- Use assistive devices like Hoyer lifts or resident lifts to ensure safe transfers and mobility support
- Document patient observations, care provided, and any incidents in compliance with healthcare standards
- Collaborate with healthcare professionals to develop and implement personalized care plans
- Provide emotional support and companionship to residents or clients, fostering a caring environment
Skills
- Proven experience in home & community care, assisted living, nursing homes, or long-term care settings
- Knowledge of patient care techniques including assisting with ADLs, medication administration, and vital signs monitoring
- Certifications such as CPR and First Aid are highly desirable
- Experience working with people with developmental disabilities, autism, dementia, or memory care needs is preferred
- Ability to work independently while following established care plans and protocols
- Strong observational skills for patient monitoring and recognizing changes in condition
- Compassionate communication skills for engaging with residents of all ages and backgrounds
- Ability to perform heavy lifting safely when required (e.g., resident transfers)
- Familiarity with HIPAA regulations to ensure confidentiality of patient information
- Experience working with specialized populations such as hospice patients or individuals requiring hospice care is a plus
