- Perform ongoing nursing assessments and monitor residents’ health and well-being
- Supervise, mentor, and guide CNAs and other direct care staff in daily resident care
- Administer medications and treatments safely and accurately, following care plans
- Complete timely and accurate documentation in accordance with LTC regulations
- Delegate appropriate tasks to team members while ensuring quality of care
- Communicate changes in residents’ condition and treatment effectiveness to the nursing team
- Educate residents and families about health, wellness, and care plans
- Support household operations and maintain a safe, comfortable environment
- Carry out additional duties as assigned by the Nurse Manager or leadership team
- Pay Range: $30.00-$34.00
- Shift Differentials: Competitive shift differentials offered
- Available Shifts: Day (7:00am – 3:00pm), Evening (3:00pm – 11:00pm), Night (11:00pm – 7:00am)
- Schedule Options: Full-Time (37.5 hours per week), Part-Time (22.5 hours per week), Weekend availability required
- Sign-On Bonus: $3,000 Sign-On Bonus for Full-Time LPNs
- Current NYS Licensed Practical Nurse (LPN) license at time of hire
- CPR certification (or willingness to obtain)
- New graduates encouraged to apply
- Experience in geriatric, rehabilitation, or long-term care preferred
- Flexibility to meet the needs of residents and the household
- Availability for rotating weekends and holidays
- Strong computer skills and accurate documentation abilities
- Excellent interpersonal and teamwork skills
- Proficient in reading, writing, and speaking English
- Ability to safely perform essential nursing functions (reasonable accommodations considered)
