We are seeking skilled, compassionate Licensed Practical Nurses (LPNs) who are passionate about geriatric care and dedicated to improving the quality of life for our long-term residents. LPN Duties and Responsibilities:

• Perform ongoing nursing assessments and monitor residents’ health and well-being

• Supervise, mentor, and guide CNAs and other direct care staff in daily resident care

• Administer medications and treatments safely and accurately, following care plans

• Complete timely and accurate documentation in accordance with LTC regulations

• Delegate appropriate tasks to team members while ensuring quality of care

• Communicate changes in residents’ condition and treatment effectiveness to the nursing team

• Educate residents and families about health, wellness, and care plans

• Support household operations and maintain a safe, comfortable environment

• Carry out additional duties as assigned by the Nurse Manager or leadership team

LPN Compensation and Schedule:

• Pay Range: $30.00-$34.00

• Shift Differentials: Competitive shift differentials offered

• Available Shifts: Day (7:00am – 3:00pm), Evening (3:00pm – 11:00pm), Night (11:00pm – 7:00am)

• Schedule Options: Full-Time (37.5 hours per week), Part-Time (22.5 hours per week), Weekend availability required

• Sign-On Bonus: $3,000 Sign-On Bonus for Full-Time LPNs

LPN Qualifications and Skills:

• Current NYS Licensed Practical Nurse (LPN) license at time of hire

• CPR certification (or willingness to obtain)

• New graduates encouraged to apply

• Experience in geriatric, rehabilitation, or long-term care preferred

• Flexibility to meet the needs of residents and the household

• Availability for rotating weekends and holidays

• Strong computer skills and accurate documentation abilities

• Excellent interpersonal and teamwork skills

• Proficient in reading, writing, and speaking English

• Ability to safely perform essential nursing functions (reasonable accommodations considered)
