Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. This role requires a strong background in addiction counseling, social work, and crisis management.
This job is for case managers, who assist people in finding housing such as : Room and boards, and independent living homes; who will benefit from placing the individuals under their case management in low income housing provided by livocity homes. Case managers are paid an incentive of 800 per placement with no limit on earnings . Contract job, workers are paid per placements in housing
Duties:
- Coordinate housing and referrals for clients
- Collaborate with community resources and agencies to ensure clients receive appropriate support
- Assist with Section 8 housing applications and support clients in finding stable housing
Experience:
- Bachelor's degree in social work or related field required; Master's degree preferred
- Minimum of 2 years of experience in case management or related field
- Strong knowledge of addiction counseling, behavioral health, and crisis management techniques
- Familiarity with Medicare guidelines and social service resources
- Excellent communication skills, both written and verbal
- Ability to work collaboratively with a diverse team of professionals
- Strong organizational skills and attention to detail
