Fresno Economic Opportunities Commission is Hiring a Case Manager Near Fresno, CA

SUPERVISORY RESPONSIBILITIES
None.
SUMMARY
Provides case management services to runaway and/or homeless youth, adults and their families through the Sanctuary Youth Shelter, Sanctuary Transitional Aged Youth (TAY) Shelter, Transitional Housing, Emergency Solutions Grant (ESG), Permanent Housing, and Rapid Re-Housing, and LGBTQ Resource Center.
Assists in the fulfillment of the agency’s vision, “For Those We Serve,” by empowering individuals to thrive as healthy, self-sufficient and contributing members of our communities.
DUTIES AND RESPONSIBILITIES/ESSENTIAL FUNCTIONS
Serves as the overall coordinator of service delivery for each participant on caseload.
Provides full support service case management to program participants including Individualized Case Plan development, regular case plan reviews and updates, ongoing needs and strength assessments, and referrals for appropriate services and resources.
Maintains case files, including documentation and tracking of all case management, counseling, and service activities.
Conducts outreach and education to the community and partnering agencies.
Confers with the Program Manager/Supportive Services Coordinator for comprehensive case plan development and service delivery.
Provides transportation and other individualized assistance as needed.
Coordinates and conducts family meetings, mediations, and/or any additional meeting regarding a program participant as needed.
Develops an operational knowledge of mainstream services, charitable organizations and local service providers to ensure clients are utilizing all services available to them.
Advocates on each client's behalf with government agencies and other service providers to ensure effective coordination of services and maximum access to resources.
Refers individuals/family members to service providers, i.e. substance abuse treatment, domestic violence services, mental health assessments, and employment training/job placement programs.
In an emergency, provides crisis intervention and/or contacts law enforcement/medical personnel. Provides and documents client follow-up and aftercare services, as applicable.
Collects, analyzes, and enters data into the Management Information System (HMIS) and/or other data systems.
Submits weekly, monthly, quarterly, semi-annual, and annual reports as directed.
Compiles statistical data required by the Program Manager to assist in contractual reporting.
Completes any child abuse referrals, if needed, as required of all mandated reporters.
Adheres to the confidentiality policy regarding clients’ personal and protected information.
Attends staff meetings and trainings as required.
YOUTH SHELTER ONLY:
  • Understands and applies Community Care Licensing/Title 22 guidelines and regulations.
  • Coordinates family reunification services or other appropriate discharge planning.
  • Conducts necessary home visits.
  • Coordinates with local school districts for transportation of youth to their school, or to arrange for delivery of homework packets.
  • Coordinates Positive Youth Development (PYD) activities, Community Services projects, and leadership opportunities for program youth.
TRANSITIONAL AGE YOUTH SHELTER ONLY:
  • Coordinates family reunification services or other appropriate discharge planning
  • Facilitates housing placement for program participants including assisting participants in applying for and securing appropriate rental housing
  • Assists participants in overcoming personal barriers to education and employment such as housing, childcare, substance abuse, and transportation issues
  • Collects and enters data on participants using MIS data system and assists participants in completing an online pre/post survey through INFORM
  • Link participants to services offered by public and private agencies
  • Conducts outreach and education to community housing providers
TRANSITIONAL HOUSING ONLY:
  • Conducts monthly program orientations and participates in program recruitment activities.
  • Conducts thorough intake interviews, verifying program eligibility including homelessness, submitting completed assessment documentation to the Program Manager
  • Assists program participants with move-ins and move-outs upon program entry and exit.
  • Researches material and conducts client life skill workshops.
  • Coordinates Quarterly Recognition Events to recognize and celebrate client achievements.
  • Conducts home visits, inspect apartments, and report maintenance concerns as needed.
  • Coordinates Positive Youth Development (PYD) activities, Community Services projects, and leadership opportunities for program youth.
EMERGENCY SOLUTIONS GRANT ONLY:
  • Submits initial assessment/applicant files to Program Manager for review to determine eligibility.
  • Facilitates housing placement for Emergency Solutions Grant program participants including assisting participants in applying for and securing appropriate rental housing.
  • Conducts outreach and education to community housing providers and act as a liaison between participants and property management companies/landlords providing units for Emergency Solutions Grant program participants.
  • Assists program participants coordinating their move-in and utilities services as required by their lease.
  • Reports on program participant challenges, successes and transitions during regularly scheduled City and County meetings, or other meetings as directed.
PERMANENT HOUSING/RAPID RE-HOUSING/ AND SUPPORTIVE SERVICES ONLY:
  • Submits initial assessment/applicant files to Program Manager for review to determine eligibility.
  • Facilitates housing placement for Permanent Housing program participants including identifying appropriate housing and facilitating lease process between landlord and Fresno EOC.
  • Conducts outreach and education to community housing providers and act as a liaison between participants and property management companies/landlords providing units for Permanent Housing program participants.
  • Assists program participants coordinating their move-in and utilities services as required by their lease.
  • Assists program participants with job placement and/or accessing appropriate benefits (i.e. disability, social security, etc.). Reports on program participant challenges, successes and transitions during Annual Performance Reviews or other meetings as directed.
LGBTQ RESOURCE CENTER ONLY:
  • Coordinates peer support groups.
  • Assists participants in overcoming personal barriers to becoming self-sufficient.
  • Collects and enters data on participants using identified data system.
  • Link participants to services offered by public and private agencies
  • Conducts outreach and education to community organizations.
Travels for agency business using personal/agency vehicle.
Maintains regular attendance and punctuality.
Works evenings and weekends as required.
Performs other duties as assigned.
QUALIFICATIONS

EDUCATION:
High school diploma or GED.
Bachelor’s Degree in Social Work, Psychology, Sociology, Counseling, Human/Child & Family Development or related field required. Work experience may substitute required education on a year-by-year basis.
EXPERIENCE:
Two years’ work experience providing case management or related supportive services to young adults and/or families required.
Prior experience working with homeless individuals and parents/families highly desirable.
OTHER QUALIFICATIONS:
Familiarity with runaway homeless services, parenting programs, service gaps, and various case management concepts and techniques.
Knowledge of child and adolescent development and causes of juvenile delinquency, substance abuse and gang involvement.
Familiarity with an ethnically/socially diverse community, and a variety of mentoring and parenting skills, concepts and techniques.
Proficient in Microsoft Office Suite and related business software.
Effective presentation, written and verbal communication skills.
Bilingual English/Spanish or other secondary language may be required.
CPR and First Aid Certification preferred, required within six (6) months of hire.
Willing to work non-traditional hours and days to meet the needs of this position.
Ability to work well with youth/teens and diverse population, and with contracted service providers and representatives of partnering agencies.
Maintain a valid California driver’s license, reliable transportation, adequate auto insurance as required by State law, and insurability by agency carrier.
Pass required background investigation including medical examination and TB clearance, acceptable criminal record statement, Department of Justice fingerprint clearance, and child abuse index clearance.
