JOB SUMMARY

The Case Manager II reports to the Supervising Case Manager and will provide supervision, direct case management, and employment services to Golden State RRC residents.

ESSENTIAL JOB RESPONSIBILITIES

  1. Maintain a caseload of Federal Bureau of Prisons (FBOP) residents.
  2. Provide new residents with a comprehensive orientation to the program.
  3. Conduct initial assessment and develop an Individual Program Plan (IPP) for assigned residents to include: physical, mental, emotional and social service needs; skill and literacy level; substance use status; family needs, basic needs.
  4. Include input and approval from the Supervising Case Manager or Program Director on resident IPPs and assessments.
  5. Educate residents and provide them with guidance to understand and follow program rules and procedures.
  6. Meet with residents no less than bi-weekly for ongoing assessment, IPP updates, assist with meeting programming goals, and to encourage progress..
  7. Make written case notes regarding resident behavior, noting significant intervention and changes in individual program plans (IPPs).
  8. Serve as a resource for residents to advise them of education and training opportunities available in the community.
  9. Provide individual and group services.
  10. Refer residents to special outside groups and seminars and verify their attendance and the quality of services.
  11. Assist residents in appropriate utilization of referral services.
  12. Obtain information necessary from resident to complete FBOP reporting requirements.
  13. Assist and schedule residents for classes and workshops.
  14. Ensure availability for one-on-on meetings, with residents, as needed.
  15. Maintain resident files to reflect FBOP contract compliance.
  16. Monitor resident accountability, review and approve passes for movement in the community, and provide suitable follow-up for infractions of rules.
  17. Monitor individuals on home confinement.
  18. Become familiar with the FBOP Statement of Work for the residential reentry center.
  19. Participate in all required FBOP scheduled trainings and program review team meetings.
  20. Provide direct job development activities and utilize community partnerships for off-site employment assistance
  21. Responsible for all initial job site approval documents.
  22. Provide home confinement and work site visits.
  23. Monitor residents work schedules, overtime, check stubs, and income deposits.
  24. Attend meetings as required by the Supervising Case Manager or Program Director.
  25. Available as a resource for the monitor staff.
  26. Maintain a caseload of FBOP residents as assigned by the Supervising Case Manager or Program Director.
  27. Drive on Agency business, as needed.
  28. Think and act quickly and efficiently in emergencies.

ESSENTIAL JOB REQUIREMENTS

  1. Four-year degree in a social, human services, criminal justice or behavioral science program from an accredited college or university and a minimum of one (1) year work experience working in human services, corrections, or community-based services. Work experience may be substituted for academic studies, exchanging one (1) year of work experience in a related field for one (1) year of academic education. Total work experience needed in lieu of the combination of education and work experience is five (5) years.
  2. Knowledge and understanding of criminal behaviors and related behavioral health issues including mental health and substance use.
  3. Two (2) years of case management experience working with justice involved individuals is preferred.
  4. Current certified training in emergency First Aid and CPR procedures, preferred.
  5. Ability to write substantive and grammatically correct reports.
  6. Ability to work with residents and staff members of diverse cultural backgrounds.
  7. Understand and follow complex oral and written instructions.
  8. Maintain accurate records, files, and databases.
  9. Operate modern office equipment, including computer equipment and specialized software application programs.
  10. Prepare, review, and present reports, recommendations, and other correspondence and communications in a clear and concise manner.
  11. Be on-site at the assigned location during regularly scheduled work hours.
  12. Must complete at least 40 hours of assigned training on an annual basis.
  13. Ability to communicate effectively in written and spoken English.
  14. Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
  15. Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  16. Ability to pass a criminal background check.
  17. Ability to pass a pre-employment drug screen and TB test.
