A very well known social services organization is looking for a part time Licensed Practical Nurse to join their growing team of Case Managers.

Job Function:

• Provides direct and supportive healthcare to individuals that reside in IAHD's residential programs
• The position focuses on coordinator of care/case management for individuals and staff training oversight for the provision of care
• Documenting individual's status, health needs and follow-up
• Attending staff meetings and participating in company functions as needed

Job Requirements:

• New York State Licensed Practical Nurse
• Experience in direct clinical work with developmentally disabled populations preferred
