Westside Family Health Center (WFHC) is a nonprofit community clinic dedicated to providing high quality, low-cost healthcare to more than 13,000 women, men, and children. Founded in 1974, WFHC is a Federally Qualified Health Center with 50 employees and volunteers. Primary Responsibilities

The Patient Care Coordinator position is responsible for accurate and timely data entry of referral requests into the appropriate portal. They are also responsible for follow-up on all specialty referrals and coordination of care for urgent referrals. The patient care coordinator is part of the care team, working as a liaison between the patient, providers, utilization management and various departments to ensure timely access to specialty care.

Qualifications
  • Two or more years of experience in customer service or one or more years of experience in a healthcare setting and prior experience in a health plan, medical group or IPA setting working with physician networks
  • Ability to convey a positive and professional image while maintaining contact with focus with audience.
  • Self-motivated, flexible and a team player, must have the ability to work independently, yet know when to ask questions.
  • Must be well organized, have excellent phone/people skills, and be able to handle multiple tasks.
  • Must have strong computer skills and effective communication skills a plus, especially in Microsoft Word, Outlook and Excel.
  • Effective verbal and written communication skills; strong computer skills desirable.
  • Available to work evening and Saturday.

Bilingual English/Spanish is preferred.

Education College or other post-secondary education preferred or work equivalent experience.
Salary and Benefits Salary is negotiable and commensurate with education and experience. WFHC offers a full range of benefits including a 403(b) plan.
