Job Summary:
We are seeking a dedicated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in promoting and improving the health and well-being of individuals and communities. You will work closely with healthcare professionals to provide education, support, and resources to individuals in need.

Responsibilities:
- Conduct outreach efforts to engage individuals and communities in health promotion activities
- Provide education on various health topics, including preventive care, chronic disease management, and healthy lifestyle choices
- Assist individuals in navigating the healthcare system, including understanding insurance options such as Medicare
- Conduct motivational interviewing to assess needs, set goals, and develop action plans for individuals
- Collaborate with healthcare providers to ensure coordinated care and follow-up for individuals
- Collect data on community health needs and trends to inform program planning and evaluation
- Assist with organizing and facilitating community health events, workshops, and support groups
- Provide addiction counseling referrals and support for individuals struggling with substance abuse
- Advocate for public health policies and initiatives that promote the well-being of the community

Skills:
- Strong background in social work or related field
- Excellent communication skills, both written and verbal
- Knowledge of medical terminology and healthcare systems
- Ability to educate individuals on health-related topics in a clear and understandable manner
- Proficient in motivational interviewing techniques to facilitate behavior change
- Comfortable with public speaking engagements and conducting presentations
- Familiarity with data collection methods and tools for program evaluation
- Understanding of addiction counseling principles and resources
- Passion for public health and improving the well-being of communities
