GENERAL FUNCTION:

The Health Care Coordinator is responsible for overseeing that each client’s healthcare needs are met in accordance with WAC, DDA Policies, and VOA-DS’ internal policies. The Heath Care Coordinator will work as part of the DS Management team and will arrange and follow up with each client’s medical appointments, prescribed medications, and overall health care. The Health Care Coordinator will be required to attend some medical appointments with clients as needed and will collaborate with client support teams to ensure care coordination remains efficient and timely.

PRINCIPAL ACTIVITIES:

• Oversight of all client health and medical needs
• Obtain and maintain all medical consents to provide healthcare treatment.
• Oversight and management of program database, tracking all client medical records.
• Receive and communicate all provider care instructions to Program Managers and their teams.
• Schedule all healthcare appointments, including annual physical, annual dental, specialty visits, follow-up appointments, therapy, and psychoactive medication reviews.
• Primary point of contact for Nurse Delegator and responsible for communicating any changes to delegated tasks and medication.
• Monitor employee NAR status and maintain current records on PHS to ensure compliance with Nurse Delegation laws.
• Attend Assessment meetings and contribute to the creation of IISP, Medical Device Plans, and relevant protocols.
• Coordinate with the Training Department to instruct new and current employees on the use of medical devices or changes in medical treatment plans.
• Complete all client’s LTC Eligibility Reviews.
• Review and audit routine medication checks completed by House Coordinators and Program Managers.
• Review MARs at the end of the month for accuracy, following up with the Program Manager to address any discrepancies.
• Attend medical appointments as needed.
• Coordinate on Incident Report follow ups where further medical follow up is required.
• All other duties as assigned by the Regional Director.

QUALIFICATIONS:

• At least 18 years of age.
• A Bachelor’s degree in a related human service field. Relevant experience may be substituted for educational requirements.
• At least one year of experience working in a Supported Living setting.
• Basic knowledge of developmental diagnoses and nutrition as it relates to health care needs.
• Knowledge of the use of medication, commonly prescribed medical devices, and Nurse Delegation laws.
• The ability to operate a computer and basic knowledge of Microsoft Suite (Word, Excel, Power Point).
• Strong organizational skills and the ability to work independently to meet deadlines.
• Ability to pass an initial, then bi-annual DSHS Background Check.
• Washington State Driver’s license and the ability to be covered by VOAWW’s vehicle insurance policy based upon an annual review of driving record.
• Strong oral and written communication skills.
