Hospice Registered Nurse, Per Diem - prior Hospice training & experience required

Job Summary:
We are seeking a compassionate and dedicated Hospice Registered Nurse to join our team on a per diem basis. As a Hospice RN, you will provide skilled nursing care to patients and their families in their homes or in a hospice facility. Your primary responsibility will be to assess patients' needs, develop and implement care plans, and ensure the highest level of comfort and quality of life for patients in their end-of-life journey.
Qualifications:
- Valid Registered Nurse (RN) license in the state of employment
- Minimum of 2 years of nursing hospice experience
- Strong knowledge of hospice care principles, including pain and symptom management
- Excellent communication and interpersonal skills to effectively interact with patients and their families
- Ability to provide emotional support and empathy to patients and their loved ones during a difficult time
- Proficient in computer documentation and electronic medical records systems
- Demonstrated ability to work independently and make sound clinical judgments
- Must possess a valid driver's license and have reliable transportation
Responsibilities:
- Conduct initial assessments of patients, including physical, psychological, and social needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' conditions, and make necessary adjustments to the care plan
- Provide direct nursing care including wound care, pain management4, and symptom control
- Educate patients and their families on end-of-life care, including pain management and comfort measures
- Coordinate and communicate effectively with the interdisciplinary team. to ensure continuity of care
- Maintain accurate and timely documentation of patient care0, including assessments, interventions, and outcomes- Provide emotional support and counseling to patients and families, promoting dignity and respect
- Participate in bereavement support services for families following the death of a loved one
