Key Responsibilities:

• Develop and oversee quality improvement for patient care and provider productivity
• Work with the executive team to minimize organizational risk
• Manage clinical measures and population health for optimal outcomes
• Facilitate full chart audits, data analysis, and reporting
• Lead chronic disease care improvement efforts
• Train and onboard new providers
• Ensure timely documentation and billing accuracy
• Visit sites monthly and facilitate meetings
