Valley Oaks Medical Group is Hiring a Patient Engagement Coordinator Near Las Vegas, NV

Job Description:

A Patient Engagement Coordinator is a healthcare professional responsible for facilitating and improving patient engagement within a healthcare setting. Their primary role is to ensure that patients are actively involved in their own healthcare, resulting in better outcomes, patient satisfaction, and overall quality of care.

Eligibility
A minimum of 1-year previous experience preferred.

Responsibilities

  • Developing patient engagement strategies.
  • Collaborating with healthcare professionals and management to create effective strategies and initiatives aimed at improving patient engagement and involvement throughout their healthcare journey.
  • Providing patients with information and resources to enhance their understanding of their medical conditions, treatment options, and self-management techniques.
  • Facilitating communication, acting as a liaison between patients, healthcare providers, and the care team to ensure effective and timely communication. This may include addressing patient concerns or questions, scheduling appointments, facilitating virtual consultations, or coordinating multidisciplinary care.
  • Collecting and analyzing patient feedback: Supporting self-management: Assisting patients in developing self-care plans and encouraging active participation in monitoring and managing their health condition.
  • Collaborating with the healthcare team: Working closely with healthcare providers, nurses, social workers, and other members of the care team to ensure a coordinated approach to patient care and engagement. This may involve participating in interdisciplinary meetings, case conferences, or care coordination activities.
  • Evaluating patient engagement initiatives: Assessing the effectiveness of patient engagement strategies and initiatives through data analysis and feedback. Making recommendations for process improvements or new initiatives based on the evaluation results.
  • Staying up-to-date with industry trends, research, and best practices related to patient engagement.

Requirements/Skills

  • Bachelor's degree in healthcare administration, nursing, public health, or a related field is required.
  • Relevant experience in patient engagement, patient advocacy, healthcare coordination, or a similar role is often preferred.
  • Strong knowledge of the healthcare industry, including knowledge of healthcare systems, policies, regulations, and patient-centered care models.
  • Familiarity with electronic health records.
  • Knowledge of eCW.

Pay: $20.00 - $25.00 per hour
