Upstate Family Health Center is Hiring a LPN Near Utica, NY

The Licensed Practical Nurse (LPN) functions under the supervision and guidance of the Medical Provider or Registered Nurse and participates in the multi-disciplinary process of providing care and treatment to patients and provides clinical support and technical assistance to the professional medical and other staff. The LPN is responsible to provide health services using a team-based care model to individuals, through effective partnerships with patients, their caregivers/families, community resources, and their primary care physician.

Essential Roles and Responsibilities

Activities include but are not limited to the following:

  • Prepare medical treatment room and set up patient care equipment.
  • Prepare patients for tests or treatments.

  • Use sanitation practices in health care settings.

  • Assist in examining or treating medical patients as needed.

  • Follow Medical office procedures.

  • Record medical history or data.

  • Weigh patients and take vital signs.

  • Collect specimens from patients, blood draws (venipunctures), capillary blood draws, urine and stool specimens

  • Administers vaccines and other medications as direct by provider orders.

  • Prepare sample for laboratory testing, analysis or microscope.

  • Follow patient care procedures.

  • Sterilize or disinfect instruments.

  • Maintain patient confidentiality and adhere to HIPAA guidelines.

  • Manages the care of patients in collaboration with other health professionals and members of the health care team in order to make decisions about treatment and attain health goals.

  • Provides education and coaching to patients and families towards successful self-management of their chronic disease; including medication reconciliation and management.

  • Obtains prior authorization for care through client’s insurance carrier as applicable.

Universal Duties

  •  Attends department meetings and any mandated continuing education/training programs
  • Comply with all pertinent regulatory and agency regulations and operating standards.

  • Establish a work environment that creates positive communication between supervisors and employees and assimilates new employees to the Agency’s culture, values and mission.

  • Participate in activities as part of the team that may include meetings, trainings, and committees.

  • Other duties as assigned.

Education and Experience Requirements

  • Minimum of a High School Diploma PLUS specialized NYS LPN Licensure.

  • CPR / AED / First Aid Certified.

  • 1-3 years of experience working in a clinical or community health center.

  • 1-3 years of experience working with chronic disease patients.

  • 1-3 years of experience working with EMR systems and data reporting.

  • Intermediate PC skills (Excel, PowerPoint, Word, Outlook, Share Point)

  • Ability to act calmly and effectively in an emergency situation.

  • Ability to be flexible to respond to changing demands.

  • Exceptional communication and interpersonal skills.

  • Blood Borne Pathogen Category (OSHA) – Category I – Job related tasks involve exposure or potential exposure to blood, body fluids, or tissue and tasks are a condition of employement.

Physical Demands

  • The physical demands described here are representative of those that be met by and employee successfully perform the essential functions of this job and include the ability to type and operate a personal computer with various software programs, effectively operate standard office equipment, bend, stoop, crouch, kneel, twist, balance, and work at a desk, lift and carry up to 60 pounds, and communicate (written and oral) in a clear and professional manner.

  • Establish and maintain effective work relationships with co-workers and patients, maintain regular attendance, understand and carry out a variety of oral and written instructions, have knowledge of proper English usage, grammar, punctuation, spelling, and vocabulary, have the ability to learn office principles, practices, and methods, understand filing systems, including numerical, alphabetical, and chronological, learn a variety of procedures, policies, and services of the assigned work unit or program, perform assigned duties with efficiency and accuracy and maintain confidentiality.

  • Certain deadlines and unanticipated developments may require work during evenings, weekends.

  • Ability to quickly address any emergent issues without losing focus on task at hand.
