POSITION SUMMARY/RESPONSIBILITIES

The Service Coordinator-Licensed Vocational Nurse (SC-LVN) collaborates with medical, behavioral, and social support staff within the Service Coordination team to provide a holistic evaluation of STAR PLUS Members’ individual dynamics, needs, and preferences while providing education and health-related information to the Member, the Member’s Representative, and the Member’s Support Network. Coordinates STAR PLUS Members covered and uncovered Long Term Services and Supports (LTSS) services, while meeting the Member's physical, behavioral, functional, and psychosocial needs. Complies with Community First Health Plans, Inc. (Community First) policies, procedures, and protocols for establishing and maintaining good working relationships with co-workers, Members, and guests

EDUCATION/EXPERIENCE

A minimum of three years of clinical nursing experience is preferred. At least two years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions is preferred. Data entry experience is required. Call center experience preferred. Experience with RUG/MDS/PASRR preferred

LICENSURE

Current unrestricted license in the State of Texas as an LVN is required
