Job Summary:
We are seeking a skilled and compassionate Nurse Practitioner to join our healthcare team. As a Nurse Practitioner, you will provide high-quality patient care, diagnose and treat various medical conditions, and collaborate with other healthcare professionals. Responsibilities:

• Conduct comprehensive physical examinations and medical assessments.
• Conduct in-person and telephone visits to assess patients’ need for Palliative Care services and ongoing eligibility.
• Consult with the patient's primary care physician, and other healthcare practitioners regarding the patient's care, ongoing care needs and medical management.
• Prescribe, review and adjust medications and treatments as needed in order to develop an individualized palliative plan of care as well as optimize symptom control and quality of life.
• Maintain accurate and comprehensive documentation of patient assessments, interventions, and palliative care plans.
• Provide patients and families with honest and sensitive information about the illness diagnosis, trajectory, and anticipated care needs.
• Educate healthcare professionals, patients, and families about palliative care principles, treatment options, and available support services.
• Performs other duties as arise.

Requirements:

• Valid Nurse Practitioner license in the state of Texas
• Masters Degree in Nursing from an accredited program
• Basic Life Support (BLS) certification
• Board Certified APN (as a Geriatric, Adult or Family Nurse Practitioner preferred).
• Minimum two years of experience as a nurse practitioner preferably in clinical nursing care of geriatrics, adult/family practice, or oncology.
• Experience with home health, hospice and palliative care.
• Excellent communication and interpersonal skills
