Job Description:
Provides medical screening exams making independent assessment for patients, both new and established, requiring palliative care. Coordinates plan of care with other designated medical staff, family, and hospice programs. The Palliative Care Nurse Practitioner (PCNP) will engage in independent decision-making about a patient's healthcare concerns and provide palliative care services on an as needed basis and make hospice referrals as appropriate.

5. Ability to read and comprehend English at a level necessary to follow written and oral instructions and safety precautions.
Required Experience:
Work Experience:
Three years of experience as a Registered Nurse in an acute care setting required. Experience as a Nurse Practitioner preferred.
License/Registration/Certification:
1. Current license to practice as a Registered Nurse in the state of Texas required.
2. Licensed as an Advanced Practice Nurse by the State of Texas
3. Nurse Practitioner Certified by The National Certification Corporation (NCC) or any other body accredited by the Texas Board of Nursing.
4. Current CPR, and ACLS certification required.
5. Must have DEA Number, DPS Number, and the ability to obtain an NPI Number.
Education and Training:
Master of Science in Nursing required.
