TRI COUNTY OFFICE ON AGING is Hiring a Case Coordinator Near Lansing, MI

Job Title: Case Coordinator
Reports to: Social Work Supervisor
Work Location: Lansing, MI
Base work schedule: 8 a.m. – 5 p.m., Monday-Friday, 40 hours/week, Full Time
Compensation: $24.68/hr plus a comprehensive benefits package including100% employer paid pension, generous time off and holidays.
General Responsibilities: Works within Project Choices, a managed care system designed to serve the elderly and the disabled who are at-risk of entering a nursing home by exploring their options and alternatives to institutional care. Completes in home assessments, arranges services and coordinates the Case Coordination Program, for those 60 and older. Assists with the Case Coordination waiting list and Crisis calls. Many of the duties listed below are mandated by the Michigan Department of Health & Human Services (MDHHS), Bureau of Aging, Community Living, and Supports (ACLS Bureau) Care Management Performance Objectives or part of millage funded services.
Essential Job Functions: (Reasonable accommodations will be provided, if necessary, for individuals with disabilities who can perform the essential job functions.)
  • Using the principles of Person-Centered Planning, develops person-centered service plans for each participant addressing the individual’s specific needs and desires.
  • Conducts in home assessments. Assessments include an evaluation of the participants physical and mental health, functional capabilities and limitations, informal and formal support systems, physical environment and financial resources
  • Arranges, coordinates and monitors the formal services for participants covered through the Case Coordination Program or millage funded services.
  • Maintains accurate files with all the mandatory documents for ACLS Bureau Case Coordination or millage funded services.
  • Completes follow-up of all services arranged as determined in the person-centered service plans and makes adjustments as needed.
  • Provides advocacy on behalf of participants to secure services and benefits to which they are entitled and promotes participant freedom of choice.
  • Provides social/emotional support if desired by the participants and their families (i.e. case conferences, crisis intervention, etc.).
  • Updates knowledge in gerontology and chronic illness management through attendance at agency meetings and continuing education opportunities.
  • Covers 24 hour on-call services as scheduled.
  • Advocates to ensure that participants are receiving benefits and services to which they are entitled.
  • Provides information and referral services to participants and other interested persons.
  • Acts as backup and overflow for Crisis calls.

Knowledge, Skills & Abilities:
  • Commitment to the organization’s missions and goals.
  • Computer skills sufficient to learn specific departmental software programs.
  • Ability to maintain confidential information regarding all aspects of participant, volunteer, employee and agency information.
  • Ability to work independently or as part of a team.
  • Ability to communicate effectively and establish good relationships with staff, participants, volunteers, and vendors.
  • Ability to represent the Agency in a professional manner.
  • Must be able to adjust priorities to meet deadlines in a timely manner.
  • Ability to meet department standards regarding job knowledge, participant focus, initiative, productivity, communication, teamwork, and attendance.
Requirements:
  • A Bachelor’s Degree in Human Services or related field is required.
  • Two years of experience working with the elderly is required.
  • One year of experience working with community senior resources is preferred.
  • A valid Michigan driver’s license in good standing (less than 4 points) is required.
Working Conditions:
  • The work environment varies. When in the office, safe work practices regarding office equipment, avoiding trips/falls and fire regulations are required. When traveling to or spending time at residential or medical facilities, exposure to unusual elements such as smoke, unpleasant odors, loud noises, and extreme temperatures increases.
  • Physical mobility is required for sitting, walking, standing, bending, and lifting/holding/carrying objects of up to 20 pounds. Must be able to drive. Ability to enter and access information using a computer. Must be able to communicate effectively with participants, co-workers volunteers and vendors in person and over the telephone.
  • Sensory requirements include exposure to uniform temperatures, conversational noise levels and everyday office activities.
  • Mental requirements include the ability to handle varying levels of stress.
