Hospice RN Case Manager
Hospice RN Case Manager Opportunity in Marietta, GATitan Placement Group is currently seeking a compassionate and experienced Hospice RN Case Manager to join a reputable healthcare team in Marietta, GA, a charming city located in the Atlanta metropolitan area known for its historic neighborhoods, vibrant arts scene, and excellent healthcare facilities.Salary and Benefits:
- Competitive salary starting at $80,000 - $90,000
- Health Insurance: Employer covers 75%
- Dental and Vision Insurance provided
- Paid Time Off: 19 days in year one
- 6 paid holidays
- Mileage Reimbursement: $0.50 per mile
- Cellphone Allowance: $60 per month
- Short-Term Disability: Paid by Employer
- Life Insurance coverage
- Retirement Package: 401(k) with match
- Full support staff
Responsibilities:
- Monday – Friday
- Provide hospice care services to patients
- Conduct home visits to assess patient needs and provide care
- Visiting 3 - 5 patients per day
- Collaborate with interdisciplinary healthcare team members
Requirements:
- Clear and active RN license in the state of Georgia
- Preferred experience in Hospice, Home Health, ICU, CCU, Oncology, End-of-Life Care, or MedSurg
- Compassionate and dedicated approach to patient care
