The Hospice Case Manager evaluates the needs of the patient, the resources available, and recommends the plan for the best outcome. Coordinates the overall care plan for home health/hospice patients. Being a Hospice Case Manager monitors the patient's progress and acts as a liaison between the patient, medical staff, and insurers. Collaborates with physicians and other medical staff members to facilitate the progression of care. In addition, Hospice Case Manager identifies patient care issues and makes recommendations.
