The Salvation Army USA Central Territory is Hiring a Community Health Worker Near Detroit, MI
Position Summary: The Community Health Worker will work with homeless or immediately homeless population discharged from a medical setting with a goal of stabilizing outpatient status and compliance with discharge plan management, reducing avoidable hospital and emergency department use.
Essential Responsibilities:
- Assist with the intake process and coordinate new resident orientation
- Encourage resident participation and engagement in community programs and activities
- Serve as resident liaison with all resources, assessments, and consultations
- Assist residents with understanding health plan benefits, navigating the care system, locating a doctor, establishing appointments, social services, and short and long term goals
- Coordinate transportation for residents as needed
- Develop, maintain a listing of all available resources and consult with them as needed
- Represent The Salvation Army and participate in community events
- Plan and coordinate support groups, life skills classes, group activities, and assessments
- Maintain case records and reports, participate in case reviews, provide reports as needed
- Assist residents in establishing and maintaining appropriate conduct and behavior while in residence including being available to give support to them as needed
- Perform other duties as assigned
Additional Responsibilities:
Qualifications:
Education/Experience:
- Bachelor's degree in social work or comparable human services field, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities
- Experience in a medical setting preferred
Skills, Knowledge & Abilities:
- Working understanding of homeless populations
Computer Skills:
- Proficient in Microsoft Office 365
- Working knowledge of TEAMS and SharePoint
- Willingness to learn new software as needed
Certificates and Licenses:
- Complete Safe From Harm training, and keep current as needed
- Must have and maintain a current valid chauffeurs license and pass the Salvation Army Motor Vehicle Record check (MVR)
