Brilliant Detroit Overview
In 2015 Brilliant Detroit was created to provide a radically new approach to kindergarten readiness in neighborhoods. The idea was to create a unique delivery model in our approach to early childhood development by using underutilized housing stock to create early child and family centers in neighborhoods. Brilliant Detroit homes provide holistic services for kids 0-8 predicated on evidence-based programs around health, family support, and education. Brilliant Detroit was born to create kid success neighborhoods. Today, in each location, neighbors come together for fellowship, activities and learning to assure school readiness, and provide needed support for families.

This work will be done in accordance with the core principles of our Detroit-based non-profit team:

Job Summary
Brilliant Detroit is seeking a Community Health Worker for the Osborn community. A Community Health Worker (CHW) is a frontline public health worker who is a trusted member, and has a close understanding of the community served. The primary responsibilities of the CHW include coaching individuals in the Osborn community on managing their health to promote self-sufficiency, and assisting individuals with navigating the health care system. The key components for their scope of practice include outreach and attracting individuals to the program, advocacy, education, mentoring, informal counseling, social support and empowerment, and doing so in a way that improves the quality and cultural competence of service delivery.

Responsibilities and Duties

General
  • Interact with participants in a professional and friendly manner
  • Identify and help eliminate barriers that restrict recruitment and services for the program
  • Participate in training and staff development as requested
  • Provide logistical support with listening sessions and other meetings as required
  • Serve on networking committees or work groups
  • Assist with research collection and organization as needed
  • Attend the 8 week CHW training course
  • Perform other duties as assigned
Outreach
  • Conduct outreach to families to bring them into the program
  • Implement individual and community assessments
Community Support
  • Provide resources and referrals based on the self-sufficiency model
  • Advocate for needed services on behalf of Brilliant participants
  • Identify current trends and neighborhood families’ needs
  • Build individual and community capacity by providing health knowledge
  • Provide cultural mediation when providing services
  • Provide coaching and social support through the program
  • Serve as the liaison and intermediary between health and social services and the community to facilitate access to services
Reporting/Administration
  • Complete all reports and paperwork in a timely manner
  • Enter program data into network database(s)
  • Complete billing information in a timely manner
Qualifications
  • High school diploma or GED required. Associate’s or Bachelor’s degree in Management, Communications, Public Health, Social Work, Social Services and or other related field preferred
  • Minimum 1 year experience working in other social service providers, as a social worker, in the public health field, or in health services preferred
  • Strong, demonstrated technological proficiency with Google Drive, especially Google Sheets and Microsoft
  • Strong, demonstrated technology and computer skills
  • Strong, demonstrated knowledge of the Osborn community
  • Member of the Osborn community preferred
  • Knowledge of the healthcare system
  • Vast knowledge of Medicaid preferred
  • Strong written, verbal and communication skills
  • Ability to coordinate multiple stakeholders and team members around a goal
  • Work with minimal up-front guidance and the ability to take ownership of work product

Work Conditions
  • Ability to work cooperatively with individuals of diverse racial/economic/cultural backgrounds
  • This position may require evening and/or weekend work
  • Local travel required, reliable transportation needed.
  • Must have reliable transportation for transport to client home visits
  • Ability to canvas a neighborhood and conduct home visits where there could be stairs
  • Ability to lift up to 20lbs
  • COVID Vaccinated individuals preferred
