Tapestry 360 Health (formerly known as Heartland Health Centers) is looking for Registered Nurses (RNs) who are passionate about providing high quality healthcare in the North Side communities we serve. As an integral Care Team member, Tap360 nurses assist with patient visits, providing assessment, triage, care coordination, and health information. They also provide clinical services, document care in the EMR, and assist with lab procedures and administrative tasks.

Major Responsibilities:

• Assist health center manager and providers
• Triage walk-ins and telehealth calls
• Administer medications; facilitate prescription refills
• Manages infection control, supplies, equipment, and storage
• Coordinates transitions in care
• Promptly and effectively support patient care delivery

Minimum Required Qualifications:

• Active Illinois Registered Nurse license
• Current Basic Life Support/CPR certification 
• Excellent customer service and nonverbal active listening skills

Desired Qualifications:

• Bachelor of Science in Nursing
• Bilingual
• Experience in community health, health education, career coordination and/or disease management
