Community Health Worker Job Description
Bright Star Community Outreach is seeking a dedicated Community Health Worker to serve as a liaison between our organization and the community, fostering trust and building relationships with residents and families in Chicago's South Side. The ideal candidate will focus on addressing social determinants of health and implementing strategies to improve overall well-being.

Technical Skills:
·High school diploma or equivalent is required.
· Previous experience in community outreach or related field is preferred.
·Strong interpersonal skills and ability to build rapport with diverse populations.
·Excellent communication and organizational abilities.
·Knowledge of social determinants of health and their impact on community well-being is preferred.
·Comfortable working independently and as part of a team.
·Proficiency in Microsoft Office suite and basic computer skills.
·Flexibility to work a hybrid schedule, with a minimum of 20 hours per week.
