Job Summary

• Community Health Worker's provide linkage and connections to health and social services, assist customers in development of post visit plans, and maintain record compliance.

Responsibilities

• Provides social services including, but not limited to economic, health, educational, social, and housing resources.
• Engages First Step customers to understand services they are receiving while at the facility.
• Supports and monitors care planning for customers following their evaluations at Summit Pointe facilities.
• Provide post-visit planning and ongoing engagement to support customer success.
• Maintains strong awareness and ability to navigate public welfare programming.
• Works closely with Summit Pointe programs including Peer Support Specialists, Recovery Coaches, Clinicians, Technicians, and Summit Pointe's Integrated Care team.
• Demonstrates good organizational, communication, computer, analytical and problem-solving skills.
• Maintains confidentiality and privacy within MDCH, state and federal guidelines.

Basic Qualifications

• High School Diploma
• Must Reside in Calhoun County, MI
• Qualify for and retain a Summit Pointe driving permit.
• Must support a trauma informed culture of safety to aid customers in their recovery process.
• One year of computer experience – Word, Excel, and electronic medical record preferred.

Preferred Qualifications

• Two (2) years of experience within a medical setting or behavioral healthcare setting.
• One (1) year of experience in working with youth experiencing a severe emotional disturbance.
• One (1) year of computer experience – Word, Excel, and electronic medical record software.
